万托林雾化吸入对老年慢性阻塞性肺疾病并Ⅱ型呼吸衰竭的疗效及安全性

摘　　要：目的观察氧气驱动雾化吸入万托林治疗老年慢性阻塞性肺疾病(COPD)急性加重期并Ⅱ型呼吸衰竭的疗效及安全性。方法将58例老年COPD急性加重期并Ⅱ型呼吸衰竭患者分为治疗组(32例)和对照组(26例),两组均给予吸氧、抗感染、祛痰、解痉、平喘、盐酸氨溴素雾化吸入等综合治疗,治疗组加用万托林雾化吸入,观察两组患者治疗前后症状、体征、动脉血气变化及不良反应。结果治疗组血气指标的改善优于对照组(P<0.05),治疗总有效率高于对照组(P<0.05)。两组不良反应的总发生率比较差异无统计学意义(P>0.05)。结论高流量氧气驱动雾化吸入万托林是治疗老年COPD急性加重期并Ⅱ型呼吸衰竭是有效且安全的方法。Objective To observe the efficacy and safety of oxygen driven inhalation ventolin of elderly chronic obstructive pulmonary disease（COPD） patients in acute exacerbation with type Ⅱrespiratory failure.Methods Fifty-eight cases of acute exacerbation of elderly COPD with type Ⅱ respiratory failure were divided into treatment group（32 cases） and control group（26 cases）.Two groups were given oxygen,antibiotics,expectorant,antispasmodic,antiasthma and other comprehensive treatments.Control group only inhalated ambroxol,treatment group were given inhalation of ambroxol combined with ventolin driven by high flow oxygen.The arterial blood gas was measured and clinical symptoms were assessed before and after treatment.The side effects were taken down.Results The improvement of blood gas in treatment group was better than that in control group（P0.05）,the total effective rate was higher（P0.05）.The total rate of the side effects unchanged.Conclusion High flow oxygen driven inhalation ventolin is an effective and safe method for elderly COPD patients in acute exacerbation with type Ⅱrespiratory failure.
