摘 要:目的探讨人体腰椎间盘退变与其高度降低之间的关系。方法用Frobin的失真-补偿X线测量法测量实验组98例(291个)、对照组200名(597个)腰椎间盘的高度值。依据髓核信号强度变化及有无突出的情况,将实验组291个腰椎间盘分成A^F组,比较各组腰椎间盘高度值与对照组之间的差异。结果 A组(信号无改变、无突出)、B组(信号无改变、有突出)及C组(信号轻至中度减低、无突出)腰椎间盘高度值与对照组比较差异无统计学意义(P>0.05)。D组(信号轻至中度减低、有突出)、E组(信号消失、无突出)及F组(信号消失、有突出)腰椎间盘高度值与对照组比较差异有统计学意义(P<0.05)。结论腰椎间盘退变先于其高度降低。腰椎间盘高度的降低出现于退变后期,退变早期不伴有高度降低。Objective To explore the relationship between the height reduced and the degeneration of the lumber intervertebral disc.Methods Using Frobin's distortion-compensated roentgen analysis(DCRA),the heights of 291 intervertebral discs of 98 patients in the experimental group and 597 intervertebral discs of 200 healthy subjects in the control group were analyzed.All 291 intervertebral discs in the experimental group were classified into group A—F according to the signal change of the nucleus pulposus with or without prolapse on sagittal T2WI.The mean height of each group was compared with that of the control group.Results The mean disc height in group A(no signal change,no prolapsed),group B(no signal change,prolapsed) and group C(moderate signal loss,prolapsed) were not significantly different with that of the control group(P0.05).The mean disc height in group D(moderate signal loss,prolapsed),group E(total signal loss,no prolapsed) and group F(total signal loss,prolapsed) deviated significantly from that of the control group(P0.05).Conclusion The degeneration of the lumbar intervertebral discs precedes the height reduced.The height reduced occurs in the late stage of the disc degeneration.In early stage of degeneration of the lumbar intervertebral discs,no height reduced appears.
