足底动静脉泵在下肢骨折患者术后的应用

摘　　要：目的探讨足底动静脉泵(A-V泵)预防下肢骨折患者下肢深静脉血栓(DVT)的效果。方法将68例下肢骨折术后患者随机分为实验组(32例)和对照组(36例)。实验组使用足底A-V泵预防DVT形成;对照组不使用足底A-V泵,仅予以抬高患肢及功能锻炼。术前1 d及术后第1、3、7、14天分别使用超声波测定下肢远、近端的静脉血栓,同时测量大腿、小腿的周径。结果实验组术后大腿、小腿水肿明显减轻,静脉血栓发生率低,与对照组比较有统计学意义(P<0.05)。结论下肢骨折术后使用足底A-V泵能有效预防DVT形成的发生,同时可有效促进局部及远端肢体软组织水肿的消除。Objective To study the efficacy of plantar arteriovenous pump（A-V pump） in preventing deep venous thrombosis（DVT） after lower limb fracture.Methods Totally 68 patients with lower limb fracture were randomized into experimental group（n=32） and control group（n=36）.Patients in experimental group were treated with plantar A-V pump while those in control group were asked to raise their affected limbs and do functional exercises.Proximal and distal venous thrombus of lower limbs were assessed by duplex ultrasound and circumferences of thigh and calf were measured on 1st day before operation and on 1st,3rd,7th,14th day after operation.Results The dropsy of thigh and calf lessened and the incidence of DVT decreased in experimental group compared with those in control group.Statistical differences were found between control group and experimental group（P0.05）.Conclusion Plantar arteriovenous pump is effective in preventing DTV after lower limb fracture and it is conducive to alleviating dropsy of the soft tissue in local and at distal end.
