摘 要:目的在动物实验基础上,将几丁糖应用于临床,观察应用前后扩张皮瓣纤维包膜中毛细血管数量的变化,探讨几丁糖对扩张皮瓣血运的影响。方法自2006年9月至2008年3月,选取24例适宜患者,即病损面积约为5em×10cm,且其周围可同时放置2个100ml的扩张器。行皮肤软组织扩张术时,在邻近病损部位两侧正常皮肤处的皮下浅筋膜深层剥离足够大的腔隙后,埋置100ml皮肤软组织扩张器各1个。将应用几丁糖的一侧设为实验组,另一侧设为对照组,并做标记。取材制作CD34染色切片后,每组随机选取15个视野于100倍的光镜下观察,计数毛细血管的数量。结果实验组毛细血管数量平均每视野为(8.200±2.150)个,对照组为(7.900±1.729)个,两组间比较其差异无统计学意义(P〉0.05)。结论在应用几丁糖干预扩张皮瓣中纤维包膜的形成过程时,不会减少纤维包膜中毛细血管的数量,因此,不会影响扩张皮瓣的血供与营养。Objective To explore the influence of Chitose on blood circulation in the fibrous capsule of extended flap by observing the changes of capillary vessel quantity based on animal experiments. Methods From September 2006 to March 2008, 24 suitable patients were selected, whose lesion area was about 5 cm x 10 cm and two 100 ml dilators could be implanted around it at the same time. During the soft tissue expansion, the deep sub- cutaneous fascia under normal skin bilateral the lesion area was dissected to form the two big lacunas for having the dilators implanted. One side with Chitosan application was as the experimental group and the other side was as the control group. Both sides were clearly marked. Slides were made and then performed the CD34 staining was performed. Fifteen fields of view were randomly selected under 100 times light microscope and the number of capillaries was counted. Results The average number of capillaries in the experimental group was 8. 200 +2. 150 on each field of view, which in the control group was 7. 900 + 1. 729. There was no significant difference ( P〉 O. 05 ) between the experimental group and the control group. Conclusion During the forma- tion of fibrous capsule of the extended flap, Chitosan does not reduce the number of capillaries in the fibrous capsule, thus there is no influence on the blood circulation and nutrition of fibrous capsule after expansion.
