丙泊酚、咪达唑仑分别复合舒芬太尼用于无痛胃镜检查的临床观察

摘　　要：目的分别研究丙泊酚、咪达唑仑复合舒芬太尼在清醒镇静胃镜下的效果、安全性,并对两者的镇静效果作一对比。方法ASAⅠ-Ⅱ级要求行门诊无痛胃镜检查的患者120例,随机分为两组,丙泊酚复合舒芬太尼组(A组)及咪达唑仑复合舒芬太尼组(B组),每组在注射丙泊酚和咪达唑仑前10min分别注射舒芬太尼0.1μg/kg,待患者睫毛反射消失后插入胃镜,检查过程中连续监测HR、MAP、SpO2,记录苏醒时间、定向力恢复时间、胃镜操作时间、术中知晓发生率等,观察胃镜检查中呛咳、体动反应、头晕的情况。结果A、B两组检查中MAP、HR、SpO2均有下降,但B组下降较为明显(P<0.05);A组患者较B组苏醒时间长,组间比较有差异(P<0.05);A组检查费用明显低于B组。结论丙泊酚复合舒芬太尼及咪达唑仑复合舒芬太尼在清醒镇静下用于无痛胃镜的检查是安全的、有效的,但是各有利弊,可以根据不同的人群选择不同的药物,使医患双方均有益处。【Objective】 To study the propofol respectively of hebron,miaow thiazole composite sufentanil in conscious sedation under the upper,and the effect and safety of both made a comparative calmness effect.【Methods】 ASA Ⅰ~Ⅱ level requirement do patients with outpatient painless gastroscope inspection,120 patients were randomLy divided into two groups,the propofol sufentanil group（A） and（of） composite cartridge sufentanil group（B）,each group in the propofol and injection of hebron miaow thiazole 10min respectively before injection of sufentanil 0.1 g/kg,stay close with eyelash disappeared into reflection,inspection process in continuous monitoring of HR,MAP,revive SpO2,record time,recovery,operation time,signicant known as observed gastroscope inspection,incidence of cough,dynamic response choked,dizzy.【Results】 A,B two groups in check SpO2 MAP,HR,has dropped,but the group B decline is obvious（P 0.05）,Group A relatively long time,revive group B groups have difference（P 0.05）.A group of examination cost significantly below group B.【Conclusion】 the propofol sufentanil miaow thiazole of hebron and composite sufentanil in conscious sedation painless gastroscope inspection for the next is safe,effective,but has its own advantages and disadvantages,can choose according to different groups of different drugs,make both good doctor-patient.
