改良型一次性无菌可控单侧支气管封堵导管和双腔支气管导管用于单肺通气的比较

摘　　要：目的比较改良型一次性无菌可控单侧支气管封堵导管(BBs)和双腔支气管导管(DLT)用于单肺通气的效果。方法 120例胸外科需在单肺通气下行择期手术患者,随机分组:DLT组,插入双腔支气管导管(60例);BBs组,插入改良型一次性无菌可控单侧支气管封堵导管(60例)。记录插管时间、肺隔离时间、定位时间及通过声门尝试次数和肺萎陷程度,术后声嘶、咽痛发生率。结果与DLT组比较,BBs组术中肺萎陷良好,术野暴露满意,且插管时间、定位时间、肺隔离时间、通过声门尝试次数、术后咽痛、声嘶发生率均降低(均P<0.05)。结论改良型一次性无菌可控单侧支气管封堵导管在单肺通气中的应用具有与双腔支气管导管相同的临床效果,使用安全、操作简便,且术后声音嘶哑、咽喉疼痛等并发症发生较双腔支气管导管更低。Objective To compare the one-lung ventilation effectiveness of the improved disposable unilateral bronchial blockers（BBs）tube and double-lumen endotracheal tube（DLT）.Methods A prospective randomized trial between the two endotracheal tubes was designed：DLT group（n=60）,improved unilateral BBs group（n=60）.The following variables were recorded：①time to intubate the assigned tube,②time to initial position of the assigned tube,③the pulmonary collapse extent,④incidence of postoperative hoarseness or sore throat.Results The time to intubation or initial position of the assigned tube and the incidence of postoperative sore throat or hoarseness in the improved unilateral BBs group were significantly reduced as compared with DLT group.Conclusion The improved disposable unilateral bronchial blockers tube is more convenient in one-lung ventilation.
