小儿发育性髋关节脱位新手术治疗方法的探讨

摘　　要：目的探讨一种新的治疗小儿发育性髋脱位手术方法。方法以选择性深度扩臼术并新型髋膝活动式支架治疗12名发育性髋脱位患儿,术后随访7～9年,观察疗效及并发症出现的情况。结果选择性深度扩臼术出血量小(66±7)ml,时间短(58±6)min,复位满意。术后患儿在短期内(3周左右)均可进行髋膝功能锻炼,无一例出现髋关节僵硬、术后再脱位及患侧股骨头无菌性坏死等并发症。结论新手术方法头臼复位确切、操作简单、创伤小、疗效较好、术后并发症少,是临床可选择的另一种新术式。Objective To evaluate a new operative technique for developmental dysplasia of the hip（DDH）.Methods 22 children with DDH were chosen for the new operative procedure of selective deep-acetabulum excavation and Salter method,and the results of them were followed up for 7 ～ 9 years.Results Less time and blood were needed for the new method.Reduction of the femoral head was good.Functional exercise was regained in a shorter time（3 weeks）.No stiffen hip and re-dislocation of the operated hip was found,and no necrosis of the femoral head occured.Conclusions The new method for the developmental dysplasia of the hip is simple,with less trauma,little blood loss,less complications after operation,etc.It is a method worthy of popularization in clinic.
