摘 要:目的研究非发酵革兰阴性杆菌烧伤分离株携带的氨基糖苷类耐药基因和整合子、转座子遗传标记。方法测定分离自烧伤患者的铜绿假单胞菌(Pa)和鲍曼不动杆菌(Ab)各20株对20种抗菌药物的敏感性,PCR检测5种氨基糖苷类修饰酶基因(aac(3)-Ⅱ、aac(6′)-Ⅰb、aac(6′)-Ⅱ、ant(3″)-Ⅰ、ant(2″),-Ⅰ)、3种16S rRNA甲基化酶基因(rmtA、rmtB、rmtD)、1种Ⅰ类整合子遗传标记(qacE△1-sul1)和1种转座子遗传标记(merA)。结果40株非发酵菌对阿米卡星、庆大霉素及妥布霉素等氨基糖苷类抗生素耐药率达到95%~100%,38株p5%,非发酵菌携带aac(6′)-Ⅱ基因,39株(97.5%)非发酵菌携带qacE△1-sul1基因。20株铜绿假单胞菌检出rmtB基因。结论本组非发酵菌烧伤分离株对氨基糖苷类抗生素耐药严重,aac(6′)-Ⅱ和qacE△1-sul1基因在非发酵菌烧伤分离株中流行。Object To investigate the aminoglycoside resistance genes and mobile genetic segments, integrons and transposons carried by nonfermenting Gram-negative bacilli isolated from burned patients. Methods Susceptibilities to 20 antibiotics of 20 strains of Pseudomonas aeruginosa and 20 strains of Acinetobacter baumannii isolated from burned patients. Genes encoding aminoglycoside-modifying enzymes(aac(3)-Ⅱ、aac(6′)-Ⅰb、aac(6′)-Ⅱ、ant(3″)-Ⅰ and ant(2″),-Ⅰ), 16SrRNA methylase(rmtA, rmtB and rmtD), integron(qacEAl-sull) and transposon(merA) were detected by PCR. Results Susceptibilities to aminoglycoside antibiotics such as amikacin, gentamycin and tobramicin were 95%-100%. 38(95%) isolates carried aac(6′)-Ⅱ and 39(97.5%) isolates carried qacEAl-sull. Futhermore, 20 Pseudomonas aeruginosa isolates carried rmtB. Conclusion 40 strains of nonfermenting Gram-negative bacilli isolated from burned patients had serious resistances to aminoglycoside antibiotics. Aac(6′)-Ⅱ and qacEAl-sull prevailed in nonfermenting Gram-negative bacilli isolated from burned patients.
