计划生育和卫生部门联合开展艾滋病自愿咨询检测和自愿咨询转诊的定性研究

摘　　要：目的了解社区计划生育部门和卫生部门的服务人员对艾滋病自愿咨询检测(HIV voluntary counseling and testing,VCT)和自愿咨询转诊(HIV voluntary counseling&referral,VCR)的知识、态度及服务开展情况,探讨社区开展VCT&VCR服务对策。方法采取方便抽样的方法,对上海市3个社区的60名社区计划生育部门和卫生部门服务人员,进行小组访谈。结果社区服务人员的艾滋病防治知识的知晓率高,但不全面。超过70%的对象不知道转诊服务,一半的人不知道免费的VCT&VCR服务。大部分人对艾滋病存有畏惧心理,但愿意提供VCT&VCR服务。现阶段开展的VCT&VCR服务主要是宣传教育服务,其次为咨询服务,技术检测服务提供率只有21.66%。服务可及性、服务质量以及社会支持环境是影响VCT&VCR服务开展的重要因素。部门合作是社区更好地开展VCT&VCR服务的重要手段。结论应进一步开展对计划生育、卫生服务人员的全面培训;同时加强部门合作,为社区提供更好的VCT&VCR服务开展环境。Objective To study the knowledge of,attitude toward and service provided on voluntary counseling testing（VCT）and voluntary counseling referral（VCR）for HIV/AIDS among community-based service providers,so as to explore a scientific strategy to conduct VCT VCR in community.Methods Using a convenience sampling strategy,60 community service providers of family planning and health sectors from three districts in Shanghai were selected to participant in focus group discussion.Results The providers under study had high-level but partial knowledge of HIV/AIDS control and prevention;more than 70 percent had no idea of VCT VCR service while only half of them heard of free service on HIV/AIDS.Though most of them were scared of AIDS,they were still willing to provide VCT VCR service if patients needed.Service related to VCT VCR provided till now were focus on public education,then on counseling,and the rate of service on HIV testing was just 21.66%.The main factors influencing the expansion of VCT VCR service were accessibility of service,ability of providers and supportive environment.The survey also showed that different departments working together might be one of the good ways to carry out the VCT VCR service better in communities by service providers of family planning and health sectors.Conclusion Comprehensive training on VCT VCR was needed for the service providers and different departments should work together.
