慢性心衰证候要素组合及其相关规律的临床文献分析

摘　　要：目的:探讨慢性心衰证候要素分布及各要素间组合的相关规律。方法:以临床文献资料为数据来源,采用一般描述性统计方法及shannon熵复杂系统划分法对数据进行挖掘和分析。结果:①慢性心衰病位在心,主要涉及肾、肺、脾、络脉和肝;阳虚、气虚、阴虚为常见病因,血瘀、水停、痰浊、饮、湿为常见病理产物。②病位类要素由单一病位到四病位组合形式,其中以两病位组合居多,以心+肾组合最为常见;病性类要素从单一要素到七要素组合形式,以气虚+阴虚、阳虚+水停、气虚+血瘀为临床常见证候形式。③相关性分析结果提示,心病及肾、肾虚为本为主要病机,气虚血瘀为基本病理环节。结论:对心衰的证候要素分布及其组合相关规律进行分析有助于把握该病证候的分布概况和基本病机特点,从而指导临床辨证治疗。Objective： To explore the syndrome factor distribution and combination laws of chronic heart failure.Methods： This paper took clinical literatures as data sources and applied generally descriptive statistical methods and shannon entropy method for data mining and analyzing.Results： ①The disease location of chronic heart failure is heart,mainly involving in kidney,lung,spleen,collaterals and liver.Yang-deficiency,qi-deficiency,and yin-deficiency are the common etiologic factors,and blood stasis,water retention,turbid phlegm,fluid retention,and dampness are the frequent pathologic products.②The syndrome factors of disease location are from single location to four-location combination and the combination of heart and kidney is the most common form.The syndrome factors of disease nature are from single factor to seven-factor combination and the combinations of qi-deficiency and yin-deficiency,yang-deficiency and water retention,qi-deficiency and blood stasis are the common syndrome forms in clinic.③Correlation analysis suggests that heart disease involving in kidney and kidney-deficiency in the root are the main pathogenesis and qi-deficiency and blood stasis are the basic pathological links.Conclusion： Analysis on the distribution and combination laws of syndrome factors of chronic heart failure can contribute to holding the general situation of syndrome distribution and the basic pathogenesis characteristic and direct the syndrome differentiation and treatment.
