摘  要:目的探讨骨内高压症的发病机理。方法随机取正常家兔16只,测量双侧胫骨近端骨内压,证明两侧压力值对比差异无显著性意义。再取家兔40只,用塑料夹板等将其一侧后肢于膝关节伸直位固定5周,制成骨内高压动物模型,死亡14只。取其中14只行双侧胫骨近端骨内压测量,证明固定侧骨内压值显著高于另(对照)侧。随机取骨内高压模型家兔12只,同时随机取正常家兔6只,对其双侧胫骨近端骨髓微循环及造血组织进行二甲基亚砜冷冻割断扫描电镜观察。结果模型家兔骨内高压侧的骨髓微循环及造血组织的形态比对照侧呈现明显差异;而正常家兔的双侧和模型家兔的对照侧对比,则无明显形态学差异。骨内高压侧的骨髓微循环广泛呈现扩张,血窦变形、破裂,内皮细胞间隙增宽,造血组织细胞水肿、粘连及异常成分的增多等病理形态学变化。结论骨髓微循环及造血组织的病理改变是骨内高压持续存在的重要原因,而骨髓微循环瘀滞则是骨内高压症的病理本质。Objective To explore the pathogenesis of intraosseous hypertension (IOH) Methods Unilateral IOH model was induced in 40 rabbits by immobilization of unilateral knee in extension with a plastic splint for 5 weeks Bilateral intraosseous pressure (IOP) was recorded in a normal group of 16 rabbits demonstrated that there was no significant difference between both sides and a subgroup of 14 rabbits demonstrated hypertension in the proximal tibiae of immobilized sides The bone marrow microcirculation and hematopoietic tissue of bilateral proximal tibiae of the IOH model of 12 rabbits and 6 normal rabbits was examined by scanning electron microscopy (SEM)with dimethyl sulfoxide (DMSO) freezecracking technique Results It was found that there was significant difference in the morphology of bone marrow microcirculation and hematopoietic tissue between IOH sides and control sides, while there was no significant difference between the control sides and both sides of the normal group A pathomorphological change was found in the bone marrow of IOH side, including the extensive dilation of bone marrow microvasculature, the deformity and destruction of sinusoid, the edema and adhesion of hematopoietic tissue etc Conclusion The results demonstrated that the pathological change of bone marrow microcirculation and hematopoietic tissue is an important factor of lasting existence of IOH,and that the stasis of the bone marrow microcirculation is pathological essence of IOH
