尿道下裂术后尿瘘的治疗

摘　　要：目的:探讨提高尿道下裂术后尿道瘘修补成功率的方法。方法:总结近几年来56例尿道下裂尿道成形术后尿瘘采取不同手术方法进行修补的临床经验,其中单发瘘32例,采用Y-V皮瓣,小瘘(<3mm)采用单纯错位多层缝合;多发瘘及较大尿瘘24例,采用"U"形皮瓣多层覆盖法进行修补。结果:一次修补成功率92.5%,其中大瘘87%、小瘘98%。结论:尿道下裂术后尿瘘修补术的手术方法多种多样,而手术方法的正确选择包括游离充足的血运良好的皮瓣,多层次的缝合等对提高手术成功率至关重要。Objective：Study and improve hypospadias postoperative urethral fistula repair rate method.Method：56 cases in recent years summerize some hypospadias urethral after angioplasty urine fistula adopt different surgical method to repair the clinical experience,including single fistula（32 cases,using Y V flap,small fistula（3mm）adopts pure dislocation multilayer suture,Multiple fistula and larger urine fistula,24 cases were adopted ＂U＂ shaped flap multilayer covering method to be repaired.Results：a repair rate joys,including fistula 87%,small fistula 98%.Conclusion：hypospadias postoperative urine fistula suture surgical method is varied,and surgical method of correct selection includes free enough blood YunLiang good flap,multi-level suture to improve the success rate is very important.
