摘　　要：目的：探讨肱骨近端内锁定系统（PHILOS）接骨板治疗肱骨近端骨折的临床疗效。方法：2005年1月~2008年6月,采用切开复位、PHILOS接骨板治疗28例肱骨近端Neer二、三、四部分骨折患者,手术采用三角肌、胸大肌间隙入路,不过分剥离附着于骨折碎片及大小、结节上的软组织下行骨折复位固定,术后次日开始练习手、腕功能,术后第三天行肩关节适量被动活动,1周后适量主动及被动活动,3周后加强主动及被动功能锻炼。结果：28例患者均获得随访,随访时间6~24个月,平均16个月。骨折愈合时间12~24周,平均愈合时间18周。按照Neer肩关节功能评分标准评定：优20例,良5例,可3例,优良率89.2%。结论：应用PHILOS接骨板可起到牢固固定肱骨近端骨折的作用,对复杂肱骨近端骨折可获得较理想的临床疗效。Objective：To investigate the clinical outcomes of the proximal humeral internal locking system（PHILOS） plate in treating proximal humeral fracture.Methods：From January2005 to June 2008,28 cases of proximal humeral fracture including Neer 2~4 parts were treated by the open reduction and PHILOS.Reduction and fixation were performed via deltopectoral-groove approach,without peeling the soft tissues of the greater and lesser tuberosity.On postoperative 1d,the hand and wrist function were begun to exercise,and the passive action of shoulder joint was properly performed.After 1 week,the proper active and passive actions were taken,and after 3 weeks,the active and passive actions were gradually strengthened.Results：28 cases were followed up for 6~24 months,average 16 months.The healing time of fracture averaged 18 weeks（12~24 weeks）.According to Neer shoulder joint function scoring,there was excellent in 20 cases,good in 5 cases,fair in 3 cases with the excellence rate of 89.2%.Conclusion：PHILOS plate can firmly fixate the fracture,provide stable fixation with good functional outcome for complex proximal humeral fracture.
