摘　　要：目的了解职业性三氯乙烯（TCE）药疹样皮炎（OMLDT）患者周围血T细胞抗原受体（TCR）Vβ亚家族基因谱系及其克隆性增殖情况。方法利用逆转录-聚合酶链反应（RT-PCR）分别检测10名健康人和5例OMLDT患者周围血中24个TCR Vβ基因的互补决定区3（CDR3）。PCR产物进一步经荧光标记和基因扫描分析CDR3长度而确定T细胞的克隆性。结果 10名健康人可检出所有24个TCR Vβ亚家族,5例OMLDT患者周围血仅检出1~6个Vβ亚家族阳性产物,平均表达（2.8±1.9）个Vβ亚家族,β亚家族检出数显著少于健康人（P〈0.01）。OMLDT患者周围血中存在克隆性增殖T细胞,其中以Vβ16亚家族出现克隆性增殖的频率较高。结论 OMLDT患者周围血中TCR Vβ谱系发生改变,这可能是接触TCE后机体细胞免疫功能的异常变化。Objective To observe the diversity of TCR Vβ gene repertoire and clonal expansion in peripheral blood mononuclear cells from 10 health individuals and 5 patients with occupational medicamentose-like dermatitis due to trichloroethylene（OMLDT）.Methods Complementarity determining region 3（CDR3） of 24 TCR Vβ genes were amplified using RT-PCR.The PCR products were further analyzed by genescan to evaluate the clonality of T cells.Results 24 Vβ subfamilies could be detected in 10 health individuals,while only 1~6 Vβ subfamilies were identified in 5 patients,showing that the detectable Vβ subfamilies in patients were significantly lower than those in the health individuals（P0.01）.Clonal expansion T cells in one or more Vβ subfamilies were found in 2 patients which were oligoclonal pattern.Conclusion Skewed distribution and clonal expansion of TCR Vβ subfamily T cells could be found in OMLDT patients.The changed repertoire of TCR Vβ T cells may be the result of the immune cytotoxicity induced by TCE.
