棘突间植入物Coflex动态固定在腰椎退行性疾病治疗中的早期临床疗效

摘　　要：目的探讨棘突间植入物Coflex在腰椎退行性疾病治疗中的短期疗效、应用指征及安全性。方法 27例腰椎退行性疾病患者在后路减压的同时行Coflex内固定术。应用视觉模拟量表(VAS)进行疼痛评估,应用Oswestry功能障碍指数评价临床疗效。结果术后随访3～18个月(平均10.5个月),VAS疼痛评分术前6.6～9.5分(平均8.5分),术后0～5.2分(平均2.8分),Oswestry功能障碍指数术前31～84分(平均67分),术后0～63分(平均35分)。无内植物松动和脱落。按中华医学会骨科学分会脊柱学组腰背痛手术评定标准评价,优16例,良8例,可3例,优良率达88.9%。结论棘突间植入物Coflex治疗腰椎退行性疾病时近期临床症状改善效果良好,是一种可供选择的非融合性固定方法。Objective To evaluate the effects of Coflex interlaminar-interspinous implant on the treatment of degenerative lumbar diseases.Methods From September 2008 to May 2010,27 patients with lumbar degenerative disease were subjected to Coflex interlaminar-interspinous implant,including 14 men and 13 women with average age of 47 years （37-61 years）.The diagnosis included degenerative spinal stenosis （n =17）,degenerative lumbar discs （n =5） and recurrent intervertebral lumbar disc hemiation （n =1）.Results The mean follow-up period was 10.5 months （range 3-18 months）.The visual analogue scale （VAS） was decreased from preoperative score of 8.5 （6.6 to 9.5） to postoperative one of 2.8 （0 to 5.2）.The patients’ Oswestry disability index was improved from preoperative score of 67 （31 to 84） to postoperative one of 35 （0 to 63）.There was no patient with implant failure.According to clinical outcome judgement criterion of surgical treatment for low back pain formulated by the spine group of Chinese Orthopaedic Association,there was excellent outcome in 16 cases,good in 8 cases,and fair in 3 cases,with the excellent and good rate of 88.9%.Conclusion Coflex interlaminar-interspinous implant could preserve the motion of stabilized segments and provide clinical improvement in patients with degenerative spinal stenosis.
