AECOPD证型、痰色与气道炎症、病情严重程度关系的研究

摘　　要：目的:对AECOPD不同证型、痰色、病情严重程度患者气道炎症程度的变化及其关系进行研究。方法:选择AECOPD患者中属于痰浊蕴肺和痰热壅肺两种证型者并进行病情严重程度分级,收集痰液与标准色谱比对进行评分,同时测定痰液中的IL-8、TNF-α水平,进行统计学分析,以P<0.05为有统计学意义的标志。结果:(1)不同证型患者IL-8水平无差异(P>0.05),TNF-α水平的差异具有统计学意义,痰热壅肺者高于痰浊蕴肺者(P<0.05)。(2)不同痰色组的IL-8、TNF-α水平的差异具有统计学意义(P<0.05)。(3)不同病情严重程度组的IL-8、TNF-α水平的差异具有统计学意义(P<0.05)。(4)痰色和病情严重程度具有相关性(P<0.05,RS=0.619)。(5)不同证型患者中其病情严重程度的构成有差异(P<0.05)。(6)痰色评分、病情严重程度与IL-8、TNF-α水平均具有相关性。结论:对于AECOPD患者,痰热壅肺者气道炎症水平可能高于痰浊蕴肺者,且痰热壅肺组中病情较重者较痰浊蕴肺者为多。从痰的颜色可以大致判断AECOPD患者的病情严重程度及气道炎症程度。Objective：To investigate the relations between AECOPD syndrome,sputum color,airway inflammation and the severity.Methods：Choose the AECOPD patients of tanzhuoyunfei and tanreyongfei and graded them based on the degree of severity,collecte sputum and score them with colour standard,then measurement the IL-8、TNF-α level of them and the results were statistically analyzed,P〈0.05 for statistical significance signs.Results：（1） there is no significant difference between two groups of IL-8 levels（P〉0.05）,TNF-α level differences were statistically significant（P〈0.05）,the goup of tanreyongfei is higher.（2） there is significant differences between different sputum colour groups of IL-8 and TNF-α levels,differences were statistically significant（P〈0.05）.（3） the differences of IL-8 and TNF-α level between the severity of different groups were statistically significant（P〈0.05）.（4） there is positive correlation between sputum color and the illness severity（P〈0.05,Rs = 0.619）.（5）the differences of constituent ratio of the degree of illness severity in different groups of tanzhuoyunfei and tanreyongfei was statistically significant（P〈0.05）.（6） there is positive correlation between sputum color,illness severity and IL-8 and TNF-α levels（P〈0.05）.Conclusion：（1） for AECOPD patients,the airway inflammation severity of tanreyongfei may be higher than the level of tanzhuoyunfei.（2） from the color of sputum,we could roughly speculate the severity of airway inflammation.
