射频靶点消融联合胶原酶治疗腰椎间盘突出症临床研究

摘　　要：目的观察射频靶点消融联合胶原酶治疗腰椎间盘突出症的临床效果,并与单纯胶原酶治疗进行对比。方法选取2007年3月-2009年10月在沈阳军区兴城疗养院全军软伤治疗与康复研究中心治疗的腰椎间盘突出患者120例,随机分为治疗组与对照组各60例,治疗组患者给予射频靶点消融及胶原酶治疗,对照组患者仅予胶原酶治疗;于治疗后1个月、6个月及12个月对两组的日本骨科协会下腰痛评分表(JOA评分),综合疗效及两组治疗安全性进行评估。结果两组患者治疗前基线资料和JOA评分相近;治疗后1个月、6个月及12个月,两组患者主要症状、临床体征和活动受限均较治疗前明显改善,JOA评分均获明显增加,且以治疗组改善较显著;治疗组优良率及有效率亦显著高于对照组。仅对照组9例患者术后2～3d出现与术前相仿的腰腿部疼痛,其余未见不良反应及并发症。结论射频靶点消融联合胶原酶治疗腰椎间盘突出症具有协同功效,能较快缓解根性神经痛症状,改善腰部活动功能,提高近期和远期疗效。Objective：To observe the effect of radiofrequency ablation with collagenase chemolysis in prolapse of lumbar intervertebral disc.Methods：From March 2007 to October 2009,120 patients were randomly allocated into 2 groups.The treatment group was received the radiofrequency ablation and collagenase chemolysis.The control group was received the single collagenase chemolysis.The changes of JOA score and comprehensive efficacy were investigated before treatment and in the 1,6 and 12 months after treatment.Results：There was no signifiance difference between two groups in the basic data analysis and the JOA score.After 1 month,6 months and 12 months in the treatment,some indices were improved including the main symptoms,the physical sign,the movement limitation and the JOA score.These improvments were more obvious in the treatment group.The excellent rate and the effective rate of the treatment group were higher than those of the control group.Conclusion：There is a cooperative effect of the radiofrequency ablation with collagenase chemolysis in prolapse of lumbar intervertebral disc.This method could relieve the neuralgia,improve the lumbar movment and increase the short-term effect and the long-term effect.
