摘　　要：目的评估Epworth嗜睡量表(ESS)对妊娠合并中、重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的临床筛查价值。方法将产妇分为单纯妊娠组、妊娠合并轻度OSAHS组、妊娠合并中、重度OSAHS组,并应用ESS进行嗜睡评分(EP),测量颈围、体质量指数(BMI),并进行多导睡眠监测(PSG)得出呼吸暂停低通气指数(AHI)和最低脉搏血氧饱和度(LSaO2),将结果行统计学处理。结果妊娠合并中、重度OSAHS组与单纯妊娠组颈围、BMI、EP值、AHI、LSaO2比较,差异均有统计学意义(P<0.05);各组BMI、颈围、EP值与AHI呈正相关,相关系数(r)分别为0.759、0.668、0.663(P<0.01);EP值、颈围、BMI三者的工作特性曲线(ROC曲线)下面积分别为0.825、0.772、0.784;当EP=7.5时,具有较高的灵敏度(76.6%)和特异性(68.2%)。结论 ESS对于妊娠合并中、重度OSAHS的孕妇更具有一定的筛查价值。Objective To evaluate clinical application of the Epworth sleepiness scale（ESS） in pregnant women with obstructive sleep apnea-hypopnea syndrome（OSAHS）.Methods Pregnant women with OSAHS were divided into three groups：pregnancy group,pregnancy with mild OSAHS group,pregnancy with moderate to severe OSAHS group.The ESS were used to count sleepiness score（EP）.Neck circumference,body mass index（BMI） and polysomnography（PSG） were measured.Then the results of apnea hypopnea index（AHI） and lowest oxygen saturation（LSaO2） were obtained.The results were conduct with statistical comparison.Results The parameters（neck circumference,BMI,EP,AHI,LSaO2） of the pregnancy with moderate to severe OSAHS group and the normal pregnancy group had significant differences（P0.05）;There were positive relationship between the parameter（BMI、NC、EP、AHI）of each group.The correlation coefficient（r） were respectively 0.759,0.668,0.663（P0.01）;The ROC curves of the parameter（EP,NC,BMI） were respectively 0.825,0.772,0.784;When the EP equal to 7.5,there were high sensitivity（76.6%） and specificity（68.2%）.Conclusion The ESS has some screening value in pregnant women with moderate to severe OSAHS.
