摘 要:目的通过对宜兴地区40岁以上人群的高血压患病情况进行流行病学调查,探讨C-反应蛋白(CRP)与血压水平及高血压的关系。方法采用分层整群抽样的方法,选择宜兴市徐舍镇5个村40岁以上的人群2 400人作为研究对象,采用统一的流行病学调查表收集基本人口学资料、生活习惯、环境因素暴露史和个人疾病史等相关信息,同时测量血压、身高、体重和腰围,检测血糖、血脂和CRP等生化指标。结果 (1)实际参与调查人数为1 899人,应答率为79.13%,收集有效问卷1 897份,有效率为99.89%;(2)线性相关分析显示,CRP与收缩压、舒张压之间无统计学联系(r=-0.023,P=0.316;r=-0.027,P=0.246);t检验结果显示,CRP升高组(≥3.23 mg/L)与CRP正常组(<3.23 mg/L)间收缩压和舒张压水平亦无显著差异(t=1.224,P=0.221;t=0.266,P=0.790)。进一步进行Logistic回归分析发现,校正年龄后,在体质指数正常(18.5~23.9 kg/m2)人群中,CRP与高血压关联的OR(95%CI)为1.509(0.981~2.230)。结论 CRP升高与收缩压、舒张压无线性关联,而体质指数正常的人群中,CRP升高可能是高血压的独立危险因素。Objective To investigate the correlation between serum C-reactive protein(CRP) and blood pressure level and hypertension.Methods A stratified cluster sampling method was conducted to select 2 400 rural residents over the age of 40 from five villages in Xushe of Yixing City;a standard epidemiological questionnaire was used to collect demographic information,life habits,environmental exposure history and personal medical history information,blood pressure,height,weight,waist circumference,blood glucose,blood lipids,CRP and other biochemical index were measured.Results (1)1 899 questionnaires were collected with the replying rate of 79.13% and 1 897 questionnaires were effective with the effective rate of 99.89%.(2)The results showed that neither significant linear association between CRP and SBP and DBP(r=-0.023,P=0.316;r=-0.027,P=0.246),nor significant difference of SBP and DBP was found between CRP elevation group(≥3.23 mg/L) and normal group(〈3.23 mg/L)(t=1.224,P=0.221;t=0.266,P=0.790).Further Logistic regression analysis indicated that there was a significant association between CRP elevation and hypertension in the population with normal BMI(18.5~23.9 kg/m2) after adjusting for age,odds ratio(95% CI) was 1.509(0.981~2.230).Conclusions Our finding suggests that there is no significant linear correlation between CRP and SBP and DBP,but CRP's elevation might significantly increase the risk of hypertension in the population with normal BMI.
