承德市社区糖尿病现状及综合干预效果分析

摘　　要：目的初步推测我市社区人群糖尿病患病率变化趋势,并对社区糖尿病患者进行干预,为今后在社区有针对性地开展糖尿病的健康教育及制订有效的预防对策与措施提供科学依据。方法随机抽取门诊健康体检人群425人相关信息进行分析。对自愿受试的110人,利用社区卫生服务站这一平台从糖尿病基本知识、饮食、运动、糖尿病药物治疗、监测血糖的重要性、低血糖的临床表现、自我救治措施、心理教育及遵从医嘱的重要意义8个方面对受试者进行综合干预。结果我市城市居民糖尿病患病率随年龄增长而升高,50岁及以上人群患病率达到23.14%。随着身体质量指数(BodyM ass Index,BM I)的增加而升高,超重人群的患病率达19.66%,综合干预后患者糖尿病相关知识的知晓率、坚持饮食和运动治疗,坚持监测血糖、遵医行为等较教育前明显提高(P<0.01);糖化血红蛋白(HbA lc)较教育前控制达标率显著提高(P<0.01)。结论健康教育是糖尿病治疗的最基本环节,糖尿病是可控制的疾病。以社区为基础,以健康教育和健康促进为依据的社区综合防治模式是糖尿病患者健康管理的重要而有效的形式之一。Objective To speculate the trend of diabetes community prevalence rate and carry out intervention and provide basis for developing pointed health education,marking effective prevention and control measures for the future.Method Analyzed the related informations of 425 healthy people from outpatient which were taken out randomly.110 volunteers took part in the test,we developed comprehensive intervention from eight aspects,they were diabetes elementary knowledge,diet,exercise,drug therapy,the importance of monitoring blood glucose levels,clinical manifestation of diabetes,self-aid,psychological education,significance of following doctor＇s advice.Results The diabetes prevalence rate in the community of Chengde increased with age,it was 23.14% above 50 years old.The number of diabetes increased with BMI too,19.66% were diabetes of overweight.After comprehensive intervention,the behavior of awareness rate of diabetes knowledge,uphold diet and exercise therapy,monitor blood glucose levels,followed doctor＇s advice improved（P0.01）.glycosylated hemoglobin attainment rate was significantly improved after education（P0.01）.Conclusion Health education is the most basic link of diabetes treatment.Conclusions comprehensive control pattern is based on health education and health promotion activities;it＇s an important and effective form of diabetes health management.
