生物敷料治疗小儿深Ⅱ度烧伤的临床研究

摘　　要：目的探讨生物敷料治疗小儿深Ⅱ度烧伤的临床优势和提高治愈率的影响因素。方法采用辐照猪皮手术治疗小儿深Ⅱ度烧伤,分伤后48 h内手术和48 h外手术两组,每组分术后使用活血药物和不使用活血药物两组。对照组6例则采用传统保痂治疗。观察患者入院后高体温天数、创面疼痛时间天数、白细胞恢复正常天数、患者住院天数等指标。结果采用辐照猪皮治疗小儿深Ⅱ度烧伤48 h内手术患者各项指标均优于48 h外手术和对照组(P<0.05),使用活血药物可以明显缩短患者住院天数(P<0.05);伤后48 h外手术患者创面疼痛时间明显小于对照组(P<0.05)。结论生物敷料治疗小儿深Ⅱ度烧伤可以有效促进创面的康复,减少患者痛苦,同时使用活血药物可进一步促进创面的愈合。Objective To explore the clinical advantage of biological wound dressing for infantile patients with deep second degree burn and the influential factors for improving the curative rate.Methods The biological wound dressing from pigs was used to treat deep second degree burn in infantile patients.The infantile patients were divided into 2 groups,and then surgically treated with biological wound dressing within or after 48 h after burn,respectively.Then the patients in each group were divided into 2 subgroups,and then the patients in one subgroup took the hemorrheologic agents after surgery,but not in the other subgroup.Six patients in control group accepted the traditional treatment.The time of fever after hospitalization,the days of wound pain,the days of white blood cells returning to the normal,and hospitalized days were observed.Results The efficacy in infantile patients surgically treated within 48 h was better than in that surgically treated after 48 h and controls（P〈0.05）.Furthermore,the hospitalized time was shorter in patients taking hemorrheologic agents than that in the others（P〈0.05）.The time of wound pain in patients surgically treated after 48 h were significantly less than that in the controls（P〈0.05）.Conclusion The biological wound dressing for infantile patients with deep second degree burn can efficiently promote the rehabilitation and alleviate the pain.Furthermore,the simultaneous use of hemorrheologic agent may further promote the wound healing.
