摘 要:目的了解深圳市呼吸道感染患者的病原菌分布及其耐药性,指导临床合理用药。方法回顾性分析2001年1月—2010年1月深圳市宝安区西乡人民医院ICU住院呼吸道感染患者痰培养结果。结果从2656例患者中分离出1 859株病原菌,阳性率达70%;革兰阴性菌占47.98%,革兰阳性菌占38.57%,真菌占13.45%。检出菌珠以大肠埃希菌、金黄色葡萄球菌、白色念珠菌、铜绿假单胞菌、肺炎克雷伯菌为前5位。革兰阳性球菌对万古霉素、替考拉宁和利福平较为敏感,而革兰阴性杆菌对亚胺培南和哌拉西林加他唑巴坦的敏感性较高。常见革兰阳性球菌、革兰阴性杆菌的耐药性有上升趋势,且具有多重耐药性。结论在治疗感染性疾病时应进行病原学检查,依据药敏试验结果合理使用抗生素,以减少耐药菌株产生。[Objective]To study the distribution and drug resistance of pathogens in patients with respiratory tract infection in Shenzheng city,so as to guide the rational use of drugs in clinic.[Methods]The sputum culture results of patients with respiratory tract infection in Xixiang People's Hospital of Bao'an District in Shenzhen City from January 2001 to June 2010 were analyzed retrospectively.[Results]1 859 strains of pathogens were isolated from 2 656 patients with the positive rate of 70%,Gram-negative bacteria,Gram-positive bacteria and fungus accounted for 47.98%,38.57% and 13.45 % respectively.The top 5 pathogens were Escherichia coli,Staphylococcus aureus,Candida albicans,Pseudomonas aeruginosa and Klebsiella pneumoniae.Gram-positive cocci were sensitive to vancomycin,teicoplanin and rifampicin.Gram-negative bacilli were sensitive to imipenem and piperacillin-tazobactam.The drug resistance of common Gram-positive cocci and Gram-negative showed an upward tendency,and there existed the multidrug resistance.[Conclusion]It is necessary to carry out the pathogens examination in the treatment of infectious disease,antibiotics must be chosen based on the results of drug sensitivity test,so as to reduce the drug-resistant bacteria.
