摘 要:目的探讨CT引导下经皮激光椎间盘减压术(PLDD)联合胶原酶化学溶解术治疗腰椎间盘突出症的疗效和优势。方法 A组:应用半导体激光,CT扫描定位,用穿刺针插入椎间盘内,然后插入光纤,每个间隙发射激光450~1000J,每个间盘注入胶原酶300~600U,共治疗35例,40个间盘。B组:单纯使用胶原酶化学溶解术治疗的30例,34个间盘,每间盘注入胶原酶300~600U。结果 观察时间为术后3d和3个月,A组总优良率达94%,未出现感染或其他严重并发症。B组优良效率83%,术后3d内出现疼痛反跳率53%,未出现其他并发症,两组有较明显差异。结论 CT引导下PLDD联合胶原酶化学溶解术治疗具有创伤小,痛苦少,安全性高,治疗效果可靠的优点,较单纯使用胶原酶组更有效,无术后疼痛反跳,是微创治疗腰椎间盘突出症的较有效方法之一。Objective To discuss of CT- guided percutaneous laser disc decompression (PLDD) united collagenase chemical disso- lution treatment of lumbar disc herniation and advantages. Methods A: Semiconductor laser, CT scanning position ,with the needle into the intervertebral disc, then insert the optical fiber, every space launch laser 450 - 1000 J, each of the discs into the collagenase 300 -600 U, a total of 35 patients ,40 discs. B: Pure chemical dissolution using collagenase treatment of 30 patients, 34 discs, each disc injection of collagenase 300 - 600 U. Results Observed after 3 days and 3 months, a group of excellent and good rate was 94% , no infection or other serious compli- cations. Excellent efficiency was 83 % of B group, after 3 days of pain 53 % rebound rate, no other complications occurred, a more significant difference between the two groups. Conclusion CT - guided joint PLDD chemical dissolution treatment of collagenase with less trauma, less pain, high safety, efficacy, and reliability features than the simple use of collagenase group more effective, no rebound pain, minimally invasive treatment of lumbar disc herniation more effective methods.
