无创正压通气治疗高龄AECOPD合并呼吸衰竭护理干预效果分析

摘　　要：目的探讨护理干预对高龄慢性阻塞性肺疾病急性加重期(acute exacerbation chronic bbstructive pulmonary disease,AECOPD)合并呼吸衰竭患者无创正压通气治疗的的影响。方法 总结分析2008年1月～2009年12月将42例患者随机分为干预组和对照组,在两组给于常规药物加NIPPV治疗期间,干预组给予综合护理干预,对照组给于常规护理。观察两组2h、48h血气变化及病情转归。结果 两组动脉血气分析差异有统计学意义(P<0.05)。干预组21例中20例顺利完成治疗,1例改换为有创通气治疗。对照组13例顺利完成治疗,4例勉强完成治疗,2例于治疗中换为有创通气治疗,2例上机1h后不能耐受而放弃NIPPV治疗。结论 有效的护理干预提高了NIPPV治疗高龄COPD合并呼吸衰竭的治疗依从性和临床疗效,减少有创机械通气给患者带来的创伤及并发症,缩短病程。Objective To study the influence of nursing intervention on non -invasive positive pressure ventilation treatment（NIPPV） used in elderly patients with AECOPD complicated with respiratory failure. Methods From January 2008 to December 2009,42 patients were randomly divided into intervention group and control group. In the two groups were giveconventional drugs plus NIPPV treatment, The interven- tion group received comprehensive nursing intervention. The control group given conventional care. The blood gas analysis and disease prognosis were observed in 2 h ,48 h. resuhs the blood gas analysis of the two groups d^splayed statistically significant （P 〈 0.05 ）. In the intervention group ,20 patients successfully completed treatment, 1 patients changed to invasive positive pressure ventilation. In control group, 13 patients successfully completed treatment ,4 cases barely completed treatment ,2 patients changed to invasive positive pressure ventilation ,2 patients can not tolerate the NIPPV treatment to give up after 1 h. Conclusion The effective nursing intervention to improve the treatment compliance and clinical efficacy of NIPPV for AECOPD complicated with respiratory ailure, reduce invasive mechanical ventilation to patients the trauma and complications, shorten the course.
