胰腺炎患者围术期血清肿瘤坏死因子-α、白细胞介素-6、-8及C反应蛋白水平变化及意义

摘　　要：目的探讨胰腺炎患者围术期血清白细胞介素-6（IL-6）、白细胞介素-8（IL-8）、肿瘤坏死因子（TNF）-α及C反应蛋白（CRP）水平变化情况及临床意义。方法选取手术治疗的胰腺炎患者39例为研究对象（观察组），选取同期健康体检正常者39名为对照组，检测两组血清IL-6、IL-8、TNF—α及CRP水平，并分析观察组术后4h、术后1d、2d及7d各项观察指标变化情况。结果观察组术前血清IL-6[（89．87±6．52）ng/L]、IL-8[（99．76±5．74）ng／L]、TNF—α[（40．56±3．67）ng／L]及CRP[（49.82±2．85）mg／L]水平均明显高于对照组（t=7．25、7．87、5．32、6．28，均P〈0．01），而术后4h、1d及2d各项指标有所下降，但与对照组比较，差异均有统计学意义（均P〈0．05）；至术后7d，两组各项指标比较，差异均无统计学意义（均P〉0．05）。结论胰腺炎患者术前血清IL-6、IL-8、TNF—α及CRP水平明显增高，至术后7d恢复正常。Objective To study the variation of serum IL-6, IL-8,TNF-α and CRP in perioperative patients with pancreatitis. Methods 39 cases in perioperative patients with pancreatitis in our hospital from January 2008 to 2010 April were selected as observation group, and 39 healthy people were selected as control group, and the serum IL-6,IL-8,TNF-α and CRP of the observation group before the treatment and after the treatment at 4h, at the first day, at the second day and the seventh day and the control group were detected, and the results were analyzed and compared. Results The serum IL-6, IL-8, TNF-α and CRP of the observation group before the treatment were higher than those of the control group（ all P 〈 0. 01 ） there were significant differences, and the levels of the observation group at 4h, at the first day,and at the second day showed decreasing,the levels compared to those of the control group （all P 〈 0.05 ） there were significant differences, the levels of the observation group at seventh day were compared to those of the control group（ all P 〉 0. 05） there were no significant differences. Conclusion It was regulation of the variation of serum IL-6, IL-8 ,TNF-α and CRP in perioperative patients with pancreatitis,and bit could provide evidence for the treatment.
