摘  要:目的:探讨早孕绒毛组织中肿瘤坏死因子受体1(mTNFR1)的表达水平及其与不明原因早期自然流产的关系。方法:采用免疫组化技术,对31例妊娠6~10周不明原因早期自然流产患者(流产组)和30例同期妊娠的健康妇女(对照组)绒毛组织中mTNFR1的表达进行定位和半定量检测。结果:流产组绒毛间质、血管内皮、细胞滋养层及合体滋养层细胞mTNFR1表达率分别为100.0%(31/31)、80.7%(25/31)、100.0%(31/31)和90.3%(28/31),对照组分别为75.0%(18/24)、58.3%(14/24)、100.0%(30/30)和95.8%(23/24)。流产组绒毛间质mTNFR1阳性表达率明显高于对照组,差异具有显著性(P<0.01)。比较mTNFR1在两组各细胞亚群中表达强度的差异,发现流产组绒毛间质细胞的TNFR1的表达强度也明显高于对照组,差异具有显著性意义(P<0.001);流产组绒毛血管内皮细胞、合体滋养细胞和细胞滋养细胞mTNFR1的表达强度虽有增强,但与对照组比较无统计学意义(P>0.05)。结论:流产组绒毛间质细胞mTNFR1阳性表达水平升高,可能导致绒毛组织间质细胞功能发生改变或损害,破坏母胎界面免疫平衡状态,与不明原因早期自然流产的发生可能有关。Objection:To investigated the relationship between unexplained early spontaneous abortion and the expression of membrane tumor necrosis factor receptor 1 protein in chorionic villi.Methods:Thirtyone women,who had unexplained spontaneous abortion at 6-10 weeks of gestational period,were used as abortion group.The control group consisted of 30 normal pregnancy women who preferred artificial abortion at 6-10 weeks gestational period as well.Chorionic villous samples from the two groups were obtained at the time of abortion.Immunohistochemistry were used to determine the location and intensity of mTNFR1 in all the chorionic villous.Results:The expressive rate of mTNFR1 in villous stromal cells,vessel endothelial,cytotrophoblast and syncytiotrophoblast cells of abortion group were 100.0%(31/31)、80.7%(25/31)、100.0%(31/31)and 90.3%(28/31)respectively,and the control group were 75.0%(18/24)、58.3%(14/24)、100.0%(30/30)and 95.8%(23/24)respectively.The expressive rate of mTNFR1 in villous stromal cells from the abortion group was higher than that in control group(100.0% vs 75.0%,P0.01).Whereas,there were no signification difference of mTNFR1 expressive rate in vessel endothelial,cytotrophoblast and syncytiotrophoblast cells between the two groups(P0.05),immunohistochemistry results also reveal that the intensity of mTNFR1 expression in villous stromal cells from the abortion group were stronger than that in control group((P0.01).Conclusion:The over expressing mTNFR1 in villous stromal cells,can possibly cause pathological changes or tissue damage,destruct immune balance of maternal-fetal interfaces in chorionic villi stormal locally,may responsible for unexplained early spontaneous abortion spontaneous abortion.
