实时三维超声心动图对扩张型心肌病患者左房容积和功能的研究

摘　　要：目的探讨实时三维超声心动图(RT-3DE)评价扩张型心肌病(DCM)患者左房功能的价值。方法使用二维超声心动图(2DE)、RT-3DE分别测量39例DCM伴左室舒张功能轻度受损者(Ⅰ组)、23例DCM伴左室舒张功能重度受损者(Ⅱ组)和34例健康志愿者(Ⅲ组)的左房最大容积(LAVmax)、最小容积(LAVmin)及左房收缩前容积(LAVpre-A),计算左房各期的射血分数(LAEF)、扩张指数(LAEI)和最大容积指数(LAVmaxI)。结果 3组间一般情况如年龄、性别、体表面积比较差异无统计学意义(P>0.05);3组间测量结果比较差异明显,均有统计学意义(P<0.05);两种方法测量DCM患者LAVmax相关性显著(P<0.01)。结论 RT-3DE能评价DCM患者左房容积和功能,DCM患者左房容积和功能与左室舒张功能相关。To evaluate the value of real-time three-dimensional echocardiography （ RT - 3DE） in left atrial（LA） volume and function in patients with dilated cardiomyopathy . Methods Thirty - nine patients with mild LV diastolic dysfunction （group I ）, 23 patients with several LV diastolic dysfunction （group Ⅱ ）and 34 healthy subjects were enrolled in this study. Their maximum volum of left atrium （ LAVmax ）, minimum volume （ LAVmin ） and pre - atrial contraction volume （LAVpre-A） were measured,then the LA ejection fraction（LAEF） , extension index （LAEI） and maximum volum index （LAVmaxI） were calculated. Results There was no significant difference among three groups in clinical data（ P 〉 0.05 ）; There was significant difference of the results observed by RT-3DE and by 2 DE among the three groups （P 〈 0.05 ） ;There was significant correlation of 2DE and RT- 3DE in mearsuring LAVmax（ P 〈 0.01 ）. Conclusion RT- 3DE can evaluate the LA volume and function in patients with DCM, there is correlation between LV volume, function and LV diastolic function.
