复杂先天性心脏病二维彩色多普勒超声诊断的评价

摘　　要：目的探讨二维彩色多普勒超声诊断复杂先天性心脏病的特征及规律性.方法对红河州第一人民医院行超声心动图检查及辅以双氧水声学造影诊断的43例复杂先天性心脏病进行了回顾性分析,总结不同切面的声像特征,并探讨检查、分析方法.结果明确诊断了法洛氏四联征7例,法洛氏三联征2例,法洛氏五联征2例,完全性心内膜垫缺损6例,爱勃斯坦畸形6例,完全型大动脉转位5例,纠正型大动脉转位1例、右室双出口3例、右室双腔心2例、右旋心并室缺1例,永存动脉干1例,完全性主动脉弓离断1例,左位上腔静脉引流入左房1例,单心室2例,三尖瓣闭锁2例,巨大右冠状动脉左室瘘1例,其中巨大右冠状动脉左室瘘、右室双腔心、爱勃斯坦畸形各1例经手术证实.结论超声心动图检查结合右心声学造影是诊断复杂先天性心脏病的首选方法.Objective To explore the characteristic and regularity of two-dimensional color Doppler ultrasound in diagnosis complex congenital heart disease.Method We retrospectively analyzed 43 patients with complex congenital heart disease diagnosed with echocardiography examination and supplemented by hydrogen peroxide contrast imaging,summarized the imaging characteristics in different section,and explored the examination and analytical method.Results The diagnosis of tetralogy of Fallot in 7 cases,trilogy of Fallot in 2 cases,pentalogy of Fallot in 2 cases,complete type ECD in 6 cases,Ebsten＇s anomaly in 6 cases,completely transposition of great arteries in 5 cases,corrected transposition of great arteries in 1 case,double-outlet right ventricle in 3 cases,double-chambered right ventricle in 2 cases,dextroversion and VSD in 1 case,persistent truncus arteriosus in 1 case,complete transection of aortic arch in 1 case,the left superior vena cava draining into the left atrium in 1 case,single ventricle in 2 cases,tricuspid atresia in 2 cases,huge right coronary artery left ventricular fistulae in 1 case.One huge right coronary artery left ventricular fistulae,double-chambered right ventricle,Ebsten＇s anomaly in 1 case were proved by surgery in our hospital.Conclusion The combination of echocardiography and right heart angiography is the first choice in the diagnosis of complex congenital heart disease.
