精液质量参数与反复自然流产及胎儿发育停滞的相关性探讨

摘　　要：目的探讨男性精液质量参数与反复自然流产及胎儿发育停滞的相关性。方法筛选189例配偶不明原因反复自然流产或胎儿发育停滞两次以上的男性精液标本,参照WHO标准分析精子密度、精子活率和活力,涂片采用苏木清伊红染色后按照WIO严格标准进行精子形态学分析,与同期育龄男性精液质量参数进行比较。结果①复发性流产及胎儿发育停滞组年龄平均(33.72±5.55)岁,精液量(2.674±0.95)ml,精子密度(62.134±54.44)×106/ml,精子活率(50.90±26.74.)%,a级精于(21.704±l5.30)%,a+b级精子(36.87±20.35)%,正常形态精子百分率(12.56±5.68)%。其中精子活率、a级精子、(a+b)级精子及正常形态精子百分率低于WHO正常值参考标准。②与同期育龄男性比较,复发性流产及胎儿发育停滞组精液质量呈现下降趋势,但差异无统计意义。结论男性精液质量参数与反复自然流产及胎儿发育停滞之间的相关性尚不明确,男性精液质量对反复自然流产及胎儿发育停滞的影响作用尚需进一步研究。Objective To investigate the correlation between semen quality parameters and recurrent spontaneous abortion or fetal development stagnated. Methods Compared with the normal reproductive men （control group） , according to 《WHO laboratory manual for the examination of human semen and sperm-cervical mucus interaction》 , all the semen samples of 189 men whose female partners recently suffered from RSA or FDS without obvious causes （experiment group） were analyzed through CASA （WLJY9000, made by BEIJING WEILI Company）, including concentration, viability, motility and sperm morphology. Results ①The average age of experiment group was （33.72 ± 5.55 ） years old, the semen volume was （ 2.67 ± 0.95 ） ml , the sperm concentration was （62.13 ±54.44） × 10^6/ml, the sperm viability rate was ）. （50. 90 ±26. 74）% , the grade A sperm was （21.70 ± 15.30） %, the grade A and B sperm was （ 36.87 ±0.35 ） %, the percent of normal morphology sperm was （ 12.56 ± 0.68 ） %. Compared with the WHO standard, the sperm viability, grade A sperm , grade A and B sperm of the experiment group had a decreasing but not marked. ②The semen quality of the experiment group was lower than the control group, but there was no significant differences were observed. Conclusion The correlation between the semen quality parameters and recurrent spontaneous abortion or fetal development stagnated was not clearly, the exact pathogenesis from the male is still unknown and more research was needed.
