摘  要:目的研究代谢综合征(MS)患者血浆胰岛素样生长因子-1(IGF-1)及胰岛素抵抗(IR)与内皮损害的关系。方法选取满足MS诊断条件的105例患者为病例组,无MS特征37例为正常对照组,根据血浆假性血友病因子水平将病例组分为MS内皮功能正常组和MS内皮功能异常组。用ELISA法测定所有个体的血浆IGF-1水平、放免法测定空腹胰岛素,用胰岛素抵抗指数(HOMA-IR)评价IR程度。结果从正常对照组到MS内皮功能正常组再到MS内皮功能异常组,IGF-1水平逐渐下降,HOMA-IR逐渐升高,三组间差异均有统计学意义(P<0.05);IGF-1与HOMA-IR呈负相关(r=-0.245,P=0.009);Logistic回归分析显示:IGF-1水平降低和HOMA-IR水平升高是内皮损害的危险因素。结论 IGF-1水平降低和IR可能与MS患者内皮功能损害相关。Objective To investigate the relationships between insulin-like growth factor-1(IGF-1) and insulin resistance(IR) with Impairment of endotheliocyte in patients with metabolic syndrome.Methods The patients with metabolic syndrome(MS) were selected as case group,and the healthy people without the characters of metabolic syndrome were selected as control group,and then the case group was divided into normal group of MS endodermis function and abnormal group of MS endodermis function according the level of serum von Willebrand factor.Serum IGF-1 concentration was measured by ELISA,fasting insulin was measured by RIA and IR was evaluated by homeostasis model assessment(HOMA).Results The level of IGF-1 was gradually decreased from control group to normal group of MS endodermis function,and then to abnormal group of MS endodermis function,and the level of HOMA-IR was increased,there were significant differences in three groups.The serum IGF-1level was negatively correlated with HOMA-IR(P〈0.01).Logistic regression analysis results showed that low level of IGF-1 and high level of HOMA-IR were risk factors of endodermis dysfunction.Conclusion Low serum IGF-1 level and insulin resistance may correlate with endodermis dysfunction in patients with metabolic syndrome.
