沙利度胺对大鼠自体髓核移植后神经根组织学的影响

摘　　要：目的探讨髓核在引起神经根组织损伤中的作用以及沙利度胺的影响。方法选取雄性SD大鼠60只随机分成对照组、自体髓核(NP)组及沙利度胺(Tha)组,建立非压迫性大鼠椎间盘突出的动物模型,并给予沙利度胺干预,各组于造模后第4、7、14、21d切取大鼠神经根进行光镜和电镜形态学观察。结果 NP组有髓神经纤维排列不规律,髓鞘破坏明显,神经节细胞水肿,尼氏小体部分脱失,出现空泡现象较重;Tha组神经纤维排列规律,髓鞘轻度破坏,神经节细胞胞质有轻度水肿,尼氏小体有少量脱失,部分出现空泡现象。对照组无特异性改变。结论髓核诱发局部炎症反应引起神经根损伤,应用沙利度胺后可缓解髓核诱发的神经根组织损伤。Objective To study the histology damage of the nerve root after nucleus pulposus（NP）transplanted into epidural space and the effect of the Thalidomide（Tha） inhibiting the damage.Methods 60 male SD rats were randomly divided into the control group,the NP group and the Tha group.The model of rat’s slipped disk without mechanical compression was established.The nerve root of rats were obtained and detected the change of histology under the light and the electron microscope in the fourth,seventh,fourteenth and the twenty-first day after the model completed.Results In the NP group,the modulated nerve fibers arranged irregularly,modularly sheath were damaged obviously.Nissl’s body was lost partially and appeared more vacuole.Medullated nerve fibers in the Tha group arranged regularly,medullary sheath was damaged little,cytoplasm had little dropsy,and the change of Nissl’s body and vacuole were less than that in the rats of the NP group.The morphology of control group had no specific changes.Conclusion Nucleus pulposus induced the inflammatory reaction and leaded to the damage of the nerve root.The Thalidomide may alleviate nerve root damaged by the nucleus pulposus.
