高血压患者大动脉弹性与颈动脉病变的关系

摘　　要：目的探讨大动脉弹性与高血压患者颈动脉病变的相关性。方法符合高血压病诊断标准入院患者110例,所有入选患者均进行颈股脉搏波传导速度(CFPWV)检测评价大动脉弹性、颈动脉超声检查评价颈动脉病变情况。根据CFPWV水平分为两组:Ⅰ组(CFPWV<13.0 m.s-1,n=58)和Ⅱ组(CFPWV≥13.0 m.s-1,n=52),观察两组患者一般临床资料。结果Ⅱ组的年龄、PP、SBP和hs-CRP均高于Ⅰ组(P<0.05)。行单因素相关分析,年龄、SBP、PP、hs-CRP与CFPWV呈正相关。多元回归分析,PP、hs-CRP与CFPWV关系最为密切(P<0.01)。与Ⅰ组比较,Ⅱ组患者颈动脉硬化或硬化伴斑块的发生率高,差异有统计学意义。结论高血压患者大动脉弹性与颈动脉病变有关,表明CFPWV可以反映颈动脉硬化的情况,与颈动脉超声一起可以对早期动脉硬化及其严重程度做出良好的预测,在临床工作中可根据各自条件选择一种检查方法用于血管早期病变的检测。Aim To explore the relationship between aortic elasticity and the severity degree of carotid artery disease in patients with essensial hypertension.Methods A total of 110 consecutive patients were enrolled.Carotid-femoral pulse wave velocity（CFPWV）was measured to evaluate the aortic elasticity.The severity degree of carotid artery disease was detected by Carotid artery ultrasound.According to CFPWV,they were divided into 2 groups：groupⅠ（CFPWV0-13.0 m·s-1,n=58）,groupⅡ（CFPWV≥13.0 m·s-1,n=52）.The clinical characteristics were analyzed.Results Age,pulse pressure,systolic blood pressure,C-reactive protein were higher in groupⅡ than in groupⅠ（P0.05）.Univariate correlation analysis showed that age,SBP,PP,hs-CRP and CFPWV were positively correlated.Multiple stepwise regression analysis showed that hs-CRP and CFPWV were most closely related（P0.01）.Compared with groupⅠ,groupⅡ had the high incidence of carotid atherosclerosis or hardening of plaque.Conclusion Arterial elasticity and carotid artery disease related,so CFPWV may reflect the situation of carotid atherosclerosis,which along with the carotid artery ultrasound can be made a good forecast for early atherosclerosis.In the clinical conditions we can select one of them for early detection of blood vessels diseases according to the respective conditions.
