射频消融前后CK-MB、hs-CRP的测定及意义

摘　　要：目的通过观察使用温控大头电极射频消融术(RFCA)前后超敏C-反应蛋白(hs-CRP)和肌酸激酶同工酶MB(CK-MB)的变化,分析温控大头电极RFCA对心肌组织的损伤,并分析两者的变化是否存在相关性。方法对70例经全面检查均无其他器质性疾病及炎症性疾病,于术前、术后12～20 h采血,样本于当日行hs-CRP和CK-MB检测。结果所有患者术前hs-CRP与CK-MB水平分别为(1.11±1.66)mg.L-1、(8.43±3.54)IU.L-1,术后分别为(2.25±2.42)mg.L-1、(11.47±4.37)IU.L-1(P值均<0.001),术前CK-MB均正常,术后5例升高(>16IU.L-1),术后hs-CRP和CK-MB Spearman相关系数为0.223,(P<0.037);经股动脉途径34例,经股静脉途径36例,术前CK-MB均无统计学意义,术后CK-MB分别为(12.59±5.19)IU.L-1、(10.42±3.16)IU.L-1,(P<0.037)。结论温控大头电极RFCA可致轻微的心肌组织损伤及炎性反应,经股动、静脉途径行RFCA对心肌组织的损伤程度不同,且hs-CRP和CK-MB存在相关性。Aim To observe the high sensitive C-reactive protein（hs-CRP） and creatine kinase MB（CK-MB） changes in myocardial tissue injury around the temperature-controlled bulk electrode RFCA,and to analyze whether there is correlation between the changes in nature.Methods Blood of 70 patients with no other organic disease and comprehensive examination of inflammatory diseases was taken before and after 12-20 hours of RFCA,the sample line on the same day hs-CRP and CK-MB test.Results Levels of preoperative hs-CRP and CK-MB were（1.11±1.66） mg·L-1,（8.43±3.54） IU·L-1,and postoperative were（2.25±2.42） mg·L-1,（11.47±4.37） IU·L-1（P0.001）.Preoperative CK-MB were negative,Postoperative five cases positive.Postoperative after hs-CRP and CK-MB Spearman correlation coefficient was 0.223（P0.037）.34 patients were through the femoral artery,femoral vein in 36 cases.There was no significant difference in preoperative,and postoperative CK-MB were（12.59±5.19） IU·L-1,（10.42±3.16） IU·L-1（P0.037）.Conclusion The temperature-controlled bulk of RFCA can cause minor myocardial damage and inflammation,femoral vein and artery approaches for RFCA of the different degrees of myocardial injury,and hs-CRP and CK-MB correlate.
