摘  要:目的了解福辛普利对老年高血压病患者血管内皮依赖性舒张功能(FMD)和血管内皮内分泌功能的影响。方法应用高频彩色多普勒超声检测仪对82例老年高血压病患者进行FMD的无创检查了解血管内皮依赖性舒张功能,同时测定血浆内皮素1(ET-1)水平了解血管内皮内分泌功能,观察福辛普利治疗6个月前后患者FMD及ET-1水平的变化。结果福辛普利治疗6个月后老年高血压病患者肱动脉基础血管内径和硝酸甘油诱发的肱动脉内径变化率与治疗前差异无统计学意义(P>0.05),但反应性充血诱发的肱动脉内径变化率较治疗前明显增加(P<0.05),同时ET-1水平明显下降(P<0.05)。结论老年高血压病患者存在明显FMD受损和血管内皮内分泌功能障碍,福辛普利治疗可改善老年高血压病患者FMD,降低ET-1水平,改善血管内皮功能。Objective To evaluate the effect of fosinopril on vascular endothelium-dependent flow-mediated dilation(FMD) and endocrine function of the vascular endothelium in elderly hypertensive patients. Methods The high frequency color Doppler imaging was used to detect the endothelium-dependent FMD in 82 elderly hypertensive patients. ET-I was at the same time tested to evaluate the endocrine function of the vascular endothelium. The changes of FMD and ET-1 were observed before and after the treatment with fosinopril of six months. Results There was no significant difference in the basic diameter of braehial artery and its change induced by sublingual nitroglycerin in elderly hypertensive patients treated with fosinopril of six months(P 〉0.05), but the changing rate of brachial artery diameter induced by reactive hyperemia was significantly increased and ET-1 levels decreased( P 〈 0.05 ) in the patients received fosinopril. Conclusion The vascular endothelium-dependent FMD and endocrine function of the vascular endothelium was impaired in elderly hypertensive patients. Fosinopril can improve the FMD ,decrease the ET-1 levels, and recover the function of vascular endothelium.
