摘 要:目的了解肥胖儿童颈动脉参数的变化及其相关性,探讨高血压、高血脂及BMI对儿童颈动脉血管的影响,为早期预防动脉粥样硬化提供科学依据。方法选取某地9~15岁肥胖儿童43例,超重儿童37例,正常体重儿童10例。测量身高,体重,血压,血脂,血糖,高、低密度脂蛋白等,进行彩色多普勒超声检查,测量颈动脉内径、血流速度、阻力指数等,统计方法包括方差检验和相关性分析。结果9—15岁儿童颈动脉血流速度、阻力指数随着年龄的增长,收缩期峰值及舒张末期血流速度逐渐减慢,差异均有统计学意义(P值均〈0.05)。阻力指数也随着年龄的增长而有降低的趋势(t=3.196,P〈0.05)。右侧颈动脉舒张末期血流速度差异有统计学意义(t=2.685,P〈0.05),而收缩期峰值血流速度、阻力指数差异均无统计学意义(P值均〉0.05)。内中膜厚度有随年龄的增长而增加的趋势(P〈0.05),颈动脉内径差异无统计学意义(P〉0.05)。结论BMI、血压、高密度脂蛋白与颈动脉内径、血流速度、内中膜厚度有相关性。Objective To understand the changes in carotid artery in obese children and its relevance to explore the impact of high blood pressure, high cholesterol and BMI on children' s carotid artery, and to provide a scientific basis for the early prevention of athetosclerosis. Methods Forty-three cases of obese children aged 9 - 15 years, thirty-seven cases of overweight children and ten cases of normal-weight children were selected to measure height and weight, blood pressure and test fasting blood, serum lipids, serum glucose, high or low-density lipoprutein, using color Doppler ultrasound to measure carotid artery diameter, blood flow velocity, resistance index, test of variance and correlation analysis. Results 9 - 15 yearolds carotid artery blood flow velocity and resistance index increased with age, while systolic and diastolic peak flow velocity gradually slowed down, the difference was significant ( P 〈 0.05 ). Resistance index also increased with age, while a falling trend, the difference was statistically significant ( t = 3. 196,P 〈 0.05 ). Right common carotid artery end-diastolic flow velocity were different ( t = 2. 685, P 〈 0. 05 ) , while the peak systolic vdocity and resistance index did not differ significantly ( P 〉 0.05 ). The intima-media thickness increased with age, and there was no difference in carotid artery diameter( P 〉 0. 05). Conclusion BMI, blood pressure, high-density lipoprotein are correlated with carotid artery diameter, blood flow velocity and intima-media thickness.
