摘 要:目的探讨采用经食管超声心动图(TEE)血流会聚法定量二尖瓣狭窄瓣口面积的可行性。方法对20例二尖瓣狭窄患者分别进行经胸超声心动图(TTE)和TEE检查。结导15例(75/)患者能用TTE血流会聚祛计算二尖瓣瓣口面积(MVA),所有患者均能采用TEE血流会聚法计算MVA;当TTE检查混叠速度(AV)为37,29,23cm/s,TEE检查AV为34、26cm/s时,血流会聚法所测MVA值与格林公式测值无显著差异(P>0.05),并具有较好的相关性(r值分别为0.76、0.79、0.66、0.81和0.76)。结小采用血流会聚法,TEE较TTE更易于测量二尖瓣口面积,二者具有相近的适宜混叠速度。Objective To understand the feasibility to determine the mitral valve area (MVA) in patients with mitral stenosis using transesophageal echocardiographic flow convergence method. Methods Both transthoracic (TTE) and transesophageal (TEE) echocardiography inc1uding Doppler flow imaging were performed in 20 patients with mitral stenosis. Results (1) The MVA can be determined in all patients by TEE, but in only 15 patients by TTE. (2) The MVA determined by TTE flow convergence method at aliasing velocity 37, 29, 23cm/s did not significantly differ from that by Gorlin's formula (P>0.05) in 15 patients, and there were good correlations between the two methods (r=0.76, 0.79, 0.66 respectively). The MVA derived from TEE flow convergence at the velocity of 34, 26cm/s were not significantly different from that calculated by Gorlin's formula (P>0.05) in 20 patients, and correlated well with those by Gorlin's formula (r=0.78, 0.76 respectively).Conclusion It is easier and more accurate to determine MVA by TEE than by TTE using flow convergence method, the appropriate alliasing velocity for TEE is comparable to that for TTE.
