肝包虫病患者服用阿苯达唑的依从性及影响因素分析

摘　　要：目的了解肝包虫病患者服用阿苯达唑的依从性及影响因素,探讨提高治疗依从性的对策。方法采用前瞻性队列研究方法,在全自治区选取易实现监测的240例病人,随机分成2组,一组120人给予阿苯达唑乳剂治疗,另一组120人给予阿苯达唑片剂治疗,连续半年每个月采用调查问卷记录一次病人服药情况,了解病人对两种药物的依从性。结果病人服用阿苯达唑半年依从性较好者占30%;对阿苯达唑片剂的依从性好于乳剂(P<0.05);多因素logistic回归分析结果显示,影响依从性的主要因素有:怀疑免费药物的疗效、不能忍受药物副反应、自行增加剂量、用药时间太长、外出劳作或工作太忙忘记或不便服药,其OR值分别为18.164、14.203、14.521、12.769、20.017。结论应加强免费药物救治政策宣传、健康教育和心理辅导、科学合理用药指导,以提高病人对阿苯达唑药物治疗的依从性。Objective To know compliance of echinococcosis patients taking albendazole and its influencing factors,and discuss strategy for improving compliance of patients.Methods In the autonomous region,240 cases from easy surveillance areas were selected and randomly divided into two groups to carry out prospective cohort study,including 120 patients being given albendazole emulsion and another 120 patients accepting albendazole tablets.Medication compliance of the patients was recorded by questionnaire every mouth for 6 months.Results Good compliance of patients taking albendazole accounted 30%.Compliance with albendazole tablets was better than that with emulsion（P0.05）.Multivariate logistic regression analysis showed that major influencing factors were doubting efficacy of free drugs,no tolerance of medication side effects（OR=18.164）,increasing their own dosage（OR=14.203）,medication too long（OR=14.521） or too busy to forget or the outside labor（OR=12.769） or inconvenient medication（OR=20.017）.Conclusions It should be strengthened policy advocacy of free drug treatment,health education and psychological counseling,scientific guidance and rational drug use in order to enhance patient compliance with albendazole therapy.
