针刺结合康复训练对脑卒中后吞咽功能障碍的影响

摘　　要：目的:观察针刺结合康复训练治疗脑卒中后吞咽功能障碍的效果,为临床治疗吞咽功能障碍寻找最优的方法。方法:将90例脑卒中后吞咽障碍患者随机分为针刺组(A)、康复训练组(B)、针刺结合康复训练组(C),每组30人。各组患者均给予脑卒中常规处理,针刺组给予项针治疗,康复训练组给予吞咽功能训练,针刺结合康复训练组则给予项针、吞咽功能训练联合治疗。治疗前后采用洼田氏饮水试验及临床疗效评价患者吞咽功能改善情况。结果:治疗后各组患者吞咽功能评分均较治疗前显著改善(P<0.05),且C组高于A、B组(P<0.05);临床疗效比较,总有效率C组优于A组和B组(P<0.05)。结论:针刺结合康复训练可进一步提高临床疗效,改善患者吞咽功能,提高病人的生活质量。Objective：In order to find optimal method by observing the effect of acupuncture combined with rehabilitation training for treating swallow dysfunction after stroke.Methods：Ninety patients were randomly divided into acupuncture group（A）,rehabilitation group（B）,acupuncture combined with rehabilitation group（C）（n=30）.Patients in all groups were given routine treatment.In addition,the patients in acupuncture group were given the acupuncture on nuchal region,the patients in rehabilitation group were given swallow functional training,the patients in acupuncture combined with rehabilitation group were given the acupuncture combined with rehabilitation treatment.Modified Kubota-Chiaris swallow functional classification wag used to assess swallow function before and after treatment.Results：After treatment,there was a significant improvement on dysphagia（P〈0.05）,and the effects were more significant in group C than in groups A and B（P〈0.05）.Conclusion：The acupuncture combined with swallow training could act synergically to ameliorate dysphagia after stroke and improve the life quality of patients.
