2型糖尿病大鼠模型中医证型演变的实验研究

摘　　要：目的观察不同时期糖尿病大鼠模型的证型属性,探讨其证型演变。方法选用Wistar大鼠,除正常对照组普食喂养外,余均高脂高糖喂养4周后,腹腔注射链脲佐菌素(STZ)2次制备糖尿病模型大鼠。将成模大鼠按血糖水平分层后随机分为模型组、增液汤组、生脉散组、肾气丸组。每组再根据用药时间不同分为、、期。每期予相应药物干预4周后,进行血糖、血脂、胰岛素、胰高血糖素检测和病理观察。结果期肾气丸干预效果优于其他各组;期生脉散干预能显著改善各指标;期增液汤干预优于其他药物干预组。病理观察发现增液汤组胰岛细胞趋于正常,未见岛内纤维化。结论此法所形成的糖尿病模型类似于人类2型糖尿病,其成模初期(期)偏于阴阳两虚,中期(期)偏于气阴两虚,后期(期)偏于阴虚。Objective To observe the TCM syndromes of rat models of type 2 diabetes and discuss the syndrome evolution in different stages of disease. Methods Wistar rats,except for the normal control group,were fed with high-fat and high-sucrose diet for 4 weeks,and treated by intraperitoneal streptozotocin injections for model making. Those with successful model establishment were randomized into control group,Zengye Tang （fluid-increasing decoction） group,Shengmai San （pulse-restoring powder） group,and Shenqi Wan （renal-qi Pill） group. Each of them was subdivided randomly into Ⅰ,Ⅱ,and Ⅲ stage subgroups for herbal medicinal intervention at the time of 0w,4w,and 8w. After the medicinal intervention for four weeks in each stage,changes in blood sugar,blood fat,insulin,glucagons,and pathologic structure were observed. Results The effect of stage Ⅰ of Shenqi Wan group was better than that of other group. All indices of stage Ⅱ of Shengmai San group were improved significantly. The result of stage Ⅲ of Zengye Tang group was superior to that of the other groups,and the islet cells of Zengye Tang group were tending to normal,and without islet fibration. Conclusion Diabetes model made by this way is similar to the human type 2 diabetes. In its early stage （stage Ⅰ）,the syndorome is of both yin and yang deficiency; in its medium stage （stage Ⅱ）,the syndorome is of both qi and yin deficiency,and in its late stage （stage Ⅲ）,the syndorome is of yin deficiency.
