摘 要:目的研究连续性血液净化与危重病患者低磷血症的相关性。方法390例次行连续性血液净化治疗危重病患者,按血磷〉0.96mmol/L、0.60~0.96mmol/L、0.50~0.60mmol/L、〈0.50mmol/L分为正常值以上血磷及轻、中、重度低磷血症。比较治疗前日、治疗结束时(未加干预)、治疗结束次日晨7时的空腹血磷值。结果接受连续性血液净化治疗后血磷水平较治疗前显著下降(P〈0.01),且82.3%存在低磷血症。治疗时间越长,血磷下降越显著(P〈0.01)。如不加干预,治疗次日晨血磷不能恢复至治疗前水平(P〈0.05)。结论连续性血液净化治疗可以降低危重病患者的血磷水平,造成低磷血症,停血液净化后不能自行恢复到治疗前水平,给予适当磷制剂补充治疗是必要的。Objective To study the correlation of continuous blood purification and hypophosphatemia of critically ill patients. Methods 390 critically ill patients accepted continuous blood purification therapy were divided into normal or different grades of hypophosphatemia by their serum phosphorus levels, 〉 0.96 mmool/L, 0.6 - 0.96 mmol/L, 0.5 - 0.6 mmool/L, 〈 0.5 mmol/L. Their fasting serum phosphorus was taken at 7 A. M. on the day before treatment, the time when continuous blood purification finished, and the next day after treatment, and their differences were compared. Results The patients' serum phosphorus decreased during the course of continuous blood purification(P〈0. 01 ). 82.3% of the patients got hypophosphatemia. The longer the treatment time was, the lower serum phosphorus levels were(P 〈0. 01 ), and it can not restore its former level without intervention(P 〈0.05). Conclusion Continuous blood purification therapy decrease patients' serum phosphorus, induce hypophosphatemia. The serum phosphorus can not restore its former level without intervention after the treatment. It is necessary to give the patients some phosphorus medicines for treatment.
