冠心病合并代谢综合征患者血清载脂蛋白B和A1及其比值与冠状动脉病变程度的关系

摘　　要：目的探讨冠心病合并代谢综合征（MS）患者载脂蛋白（Apo）B、ApoA1及其比值与冠状动脉病变程度的关系。方法选取在我院行冠状动脉造影并确诊为冠心病的患者211例，分为MS组（99例）和非MS组（112例），其中MS组根据病变支数分为单支病变组、双支病变组、三支病变组，分别测定ApoB、ApoA1及其他危险因素，用Gensini评分系统测定冠状动脉病变程度，评价冠心病合并MS患者血清ApoB、ApoA1及其比值与冠状动脉病变程度的关系。结果与非MS组比较，冠心病合并MS组ApoB、ApoB／ApoA1均显著高于非MS组（P〈0．01），并且Gensini评分及三支病变比例高于非MS组（P〈0．05），而ApoA1水平两组比较差异无统计学意义（P〉0．05）。结论冠心病合并MS的患者ApoB、ApoB／ApoA1显著增高，且与冠状动脉狭窄程度及范围有关，ApoB、ApoB／ApoA1是评价MS的良好参数。Objective To investigate the relationship of serum ApoB, ApoA1 ApoB/ApoA1 ratio with the severity of coronary artery stenosis in the patients with coronary heart disease （CHD） complicated by metabolism syndrome （MS）. Methods A total of 211 CHD patients undergoing coronary angiography were enrolled and were divided into MS group（ n =99） and non- MS group（ n = 121 ）. The MS group was divided into the single vessel, double and triple vessels lesion groups. The severity of coronary artery stenosis was assessed by the sum of the Gensini scores. The ApoB, ApoAl and other risk factors in MS group were compared with those in non - MS group in order to assess its relationship with the severity of coronary artery stenosis. Results Compared with the non - MS group, the MS group had higher levels of ApoB, ApoB/ApoAl ratio（ P 〈 0.01 ） and higher scores of Gensini, proportion of triple vessels lesion （ P 〈 0.05 ）, but there was no significant difference in serum ApoA1 level between groups with and without MS（P 〉0. 05）. Conclusion The CHD patients with MS have higher levels of ApoB, ApoB/ApoA1 and may be related to the severity and number of coronary artery branch lesion. ApoB, ApoB/ApoA1 are the good parameters in assessing MS.
