小剂量盐酸托烷司琼经硬膜外给药对剖宫产术中呕吐的影响

摘　　要：目的:研究小剂量盐酸托烷司琼经硬膜外给药对剖宫产术中呕吐的影响。方法:150例ASA分级1/2级的择期行剖宫产术孕妇随机分为3组,均经硬膜外予1.2%盐酸利多卡因、0.25%丁卡因的混合液10 ml。待胎儿娩出后,Ⅰ组经硬膜外予盐酸托烷司琼0.5 mg,Ⅱ组经静脉给予盐酸托烷司琼5 mg,Ⅲ组为不使用任何止吐药物的对照组。结果:(1)3组孕妇术中呕吐发生率分别为4%、6%和64%,Ⅰ、Ⅱ组间无显著性差异(P>0.05),而Ⅰ、Ⅱ组与Ⅲ组比较均有显著性差异(P<0.01);(2)3组孕妇术中呕吐程度计分分别为(1.06±0.31)、(1.08±0.34)和(2.08±1.03)分,Ⅰ、Ⅱ组间无显著性差异(P>0.05),而Ⅰ、Ⅱ组与Ⅲ组比较均有显著性差异(P<0.01)。结论:盐酸托烷司琼小剂量硬膜外给药与静脉给药两种途径对剖宫产术中呕吐均有明显的抑制作用,且硬膜外复合给药所需药物剂量更少。Objective：To study the effect of small dose triopisetron hydrochloride administered epidurally during cesarean section.Methods： 150 subjects of cesarean section scaled Ⅰ/Ⅱ by AS were divided into three groups randomly.Epidural administration of 1.2% hydrochloric acid lidocaine,0.25% teracainum in an amount of 10 ml was given to all;after the delivery of the fetus,group Ⅰ was administrated epidurally with 0.5 mg hydrochloric acid tropisetron;group Ⅱ was administrated through vein with 5 mg hydrochloric acid tropisetron;group Ⅲ served as a blank group with no medication.Results：（1） The frequency of vomiting in three groups was 4%,6% and 64% respectively;no obvious difference was seen between group Ⅰ and group Ⅱ（P0.05）,but there was an obvious difference between group Ⅰ and group Ⅲ or group Ⅱ and group Ⅲ（P0.01）.（2） The scoring of vomit degree of three groups are（1.06±0.31）,（1.08±0.34）and（2.08±1.03）points;their was no obvious difference between group Ⅰ and group Ⅱ（P0.05）,but obvious difference was seen between group Ⅰ and group Ⅲ or group Ⅱ and Group Ⅲ（P0.01）.Conclusion： Small dose triopisetron hydrochloride can inhibit vomiting in cesarean section,smaller dose is needed in epidural administration compared to veinous administration.
