中西医结合分组悬吊浅剥全扎术治疗环状混合痔

摘　　要：目的探讨中西医结合分组悬吊浅剥全扎术治疗环状混合痔的临床应用价值。方法将332例环状混合痔患者完全随机分为治疗组166例、对照组166例,治疗组采用中西医结合分组悬吊浅剥全扎术治疗,对照组采用传统外剥内扎术治疗。观察临床疗效、残留痔情况、手术时间、创面愈合时间;观察术后疼痛、水肿、出血、尿潴留、肛门狭窄、肛门失禁等主要并发症的发生情况;观察2种手术对肛门直肠功能的影响。结果治疗组临床疗效优于对照组,残留痔少于对照组,手术时间和创面愈合时间均短于对照组(P<0.01)。治疗组术后主要并发症发生均低于对照组(P<0.01,P<0.05)。2组治疗后肛管最长收缩时间均较本组治疗前延长(P<0.01);治疗组治疗后直肠静息压较本组治疗前降低(P<0.05);2组治疗后肛管静息压均较本组治疗前降低(P<0.01);在直肠静息压、肛管静息压改变方面治疗组均优于对照组(P<0.05)。结论中西医结合分组悬吊浅剥全扎术,可作为治疗环状混合痔的一种更加合理有效的新术式。Objective To investigate the clinical practical value of grouped suspending and shallow stripping full ligation integrated Chinese and Western medicine in the treatment of circular mixed hemorrhoids.Methods All 332 patients were randomly divided into the treatment group（n=166） and control group（n=166）.The treatment group was treated with grouped suspending and shallow stripping full ligation integrated Chinese and Western medicine,and the control group,with traditional Milligan-Morgan operation.The curative effect,residual hemorrhoids,operation time and wound healing time were observed.The postoperative complications were also observed including pain,edema,hemorrhage,uroschesis,anal stenosis and fecal incontinence.The influences of two operations on anal and rectal functions were studied.Results In the treatment group the curative effect was better,residual hemorrhoids were less,and operation time and wound healing time were shorter compared with the control group（P0.01）.The occurrence rate of main complications was lower in the treatment group than that in the control group（P0.01,P0.05）.The anal longest contraction time was prolonged in two groups（P0.01）,the rectal resting pressure was reduced in the treatment group（P0.05）,and the anal resting pressure was reduced in two groups（P0.01） after the treatment.The treatment group was superior to the control group in the changes of rectal resting pressure and anal resting pressure（P0.05）.Conclusion The grouped suspending and shallow stripping full ligation integrated Chinese and Western medicine is a new therapy for treating circular mixed hemorrhoids,which is more reasonable and effective.
