An Uncommon Cause of Nasal Block： Sarcoidosis

摘　　要：Nasal block is a usual presentation in the otorhinolaryngology clinic. Cause are many, however the diagnosis of sarcoidosis is a rare one. We describe a case of a young man with a 5-month history of severe bilateral nasal obstruction associated with hyposmia and occasional epistaxis, despite being on nasal decongestants, topical nasal steroids and antihistamines. He also had mild dyspnea on exertion and a skin rash over the arms and trunk. Clinical examination showed hypertrophied inferior turbinates with friable nodular mucosa overlying the turbinates, nasal floor and septum bilaterally as well as the nasopharynx. Systemic examination revealed hepatosplenomegaly and a generalized papular skin lesion in varying degrees of resolution. Histopathological examination of the nasal mucosa showed fragments of inflamed respiratory mucosa consisting of multiple noncaseating epithelioid granulomas within the stroma. Sinonasal sarcoidosis may be associated with multiorgan involvement thus a complete systemic assessment is mandatory.
