摘　　要：目的:研究SPECT/CT在骨显像中的诊断价值,探讨SPECT/CT在临床实践中的临床用价值。方法:回顾性分析358例肿瘤患者的SPECT/CT骨显像结果,用五级评分法对病灶的诊断把握度进行分级,应用三级评分法分析记录诊断改变,最后综合分析SPECT/CT骨显像的诊断价值。结果:①融合显像对近50%(176/358)的患者有诊断增益,SPECT/CT对所有425处病灶的诊断增益为52%,对205例表现为单发病灶的患者的诊断增益为67.8%;②对于平面骨显像不确定的79个病灶,SPECT/CT融合显像能明确诊断87.34%的病灶,而剩余10例即在CT的帮助下依然无法确诊。结论:SPECT/CT将功能显像和解剖显像有机的结合起来,提高了诊断的准确性,增强了医生的诊断信心,在鉴别骨良恶性病变方面有重要的临床价值。Objective：To study the value of SPECT/CT in the bone scanning,and to explore the value of SPECT/Ct in the clinical applications.Methods：SPECT/CT bone scanning was performed on 358 patients.The results were recorded with a five-point scsle.And then the changes of the diagnosis were recorded using a three-point scale to evaluate the extent of the added value.Finally,we made a comprehensive analysis of the additional value of SPECT/CT bone scanning.Results：① SPECT/CT had about 50% additional value for 358 cancer patients.SPECT/CT had 52% additional value for 425 lesions.And for 205 cases who showed solitary lesion,there was the higher value in 67.8%.;② For the 79 indeterminate lesions on planar bone scan,SPECT/CT can confirm 87.34% of the lesions,while the remaing 10 cases was still not confirmed,even in the help of CT.Conclusion：SPECT/CT can combine the functional imaging with the anatomical imaging.Owing to the improved diagnostic accuracy and confidence,SPECT/CT has important clinical application in differentiating benign and malignant lesions of bone.
