开颅手术病人口服左旋氧氟沙星片的药代动力学研究

摘　　要：目的：研究开颅手术患者ｐｏ 左旋氧氟沙星片的药代动力学。方法：采用反相高效液相色谱法测定６ 名开颅手术患者单剂量ｐｏ 左旋氧氟沙星３００ｍｇ 后血浆中左旋氧氟沙星浓度，以３Ｐ８７ 药代动力学计算程序处理测定结果，计算药代动力学参数。结果：其药代动力学特性符合一室开放模型，消除半衰期、峰浓度、药时曲线下面积、血浆总的清除率分别为（６．１５０ ±１－８５２）ｈ，（３．６５４ ±０ ．３９０）ｍｇ·Ｌ－ １ ，（３３ ．６８６±８．２９４）ｍｇ·ｈ·Ｌ－１ ，（９．４９６±２ ．９４０）Ｌ·ｈ－１ 。所得结果与健康成年人单剂量ｐｏ 左旋氧氟沙星３００ｍｇ 的药代动力学参数近似，但患者的表观分布容积（７８ ．２１２ ±６ ．８７１）Ｌ明显比正常人高。结论：实验结果提示开颅手术可能使左旋氧氟沙星透过血脑屏障量增加。OBJECTIVE:To study the pharmacokinetics of levofloxacin in patients undergoing neurosurgical operation.METHODS:The plasma concentrations of levofloxacin were measured by HPLC in 6 patients undergoing neurosurgical operation after a single oral adminstration of 300mg.RESULTS:The pharmacokinetic parameters could be described by one compartment open model.The elimination half life,peak plasma concentration,area under curve,total plasma clearance were (6.150±1.852)h,(3.654±0.390)mg·L -1 ,(33.686±8.294)mg·h·L -1 ,and (9.496±2.940)L·h -1 respectively.The pharmacokinetics of levofloxacin in these patients were similar to that in normal volunteers.But the distribution volumn in these patients were significantly higher than that of volunteers.CONCLUSION:The results showed that more levofloxacin probably passed through the blood brain barrier under neurosurgical operation.
