摘  要:目的对新生儿常见疾病中血清氨基末端B型钠尿肽原(NT-proBNP)水平的变化和分组变量的相关度进行多元回归分析,建立有效的监控预测预报机制。方法采用电化学发光法检测166例新生儿(重度窒息组35例、窒息组46例、肺炎组42例、其他疾病组43例)血清NT-proBNP水平。以血清NT-proBNP检测结果为主要因素进行计量资料的回顾性分析。结果重度窒息组血清NT-proBNP水平明显高于其他3组(P<0.001);窒息组与肺炎组明显高于其他疾病组(P<0.001、P<0.05);窒息组与肺炎组无差异(P>0.05)。各组经多元线性回归检验分析差异有统计学意义(F=142.55,P<0.001)。复相关指数(R2)=0.93,校正相关指数(R)=0.93。分析得到预报方程式Y=7 578.85-1.64X1+8.40X2-4.58X3。结论血清NT-proBNP水平在各组间的变化存在差异。重度窒息患儿比其他患儿更具高危性。通过预报方程式对患儿血清NT-proBNP进行监测可以预示疾病的发展趋势。Objective To analyze the correlation between serum N-terminal pro-B-type natriuretic peptide(NT-proBNP) and the group variables,and establish an efficient mechanism to monitoring,predicting and forecasting with the help of multiple regression.Methods 166 neonates were enrolled and classified into severe asphyxia group(35 cases),asphyxia group(46 cases),pneumonia group(42 cases) and other disease group(43 cases).Serum NT-proBNP levels were measured by electrochemiluminescent immunoassay.According to the levels of serum NT-proBNP,retrospective analysis was conducted subsequently.Results Among the 166 neonates,the serum NT-proBNP levels of severe asphyxia patients were obviously higher than those in other three groups(P0.001).The levels of serum NT-proBNP in asphyxia group and pneumonia group were significantly higher than other disease group(P0.001,P0.05).There was no significant difference between asphyxia group and pneumonia group(P0.05).By multiple regression analysis,the differences among these groups were significant(F=142.55,P0.001).Multiple correlation coefficient(R2) was 0.93,and the correction index(R) was 0.93.The prognostic equation obtained by analysis was Y=7 578.847-1.64X1+8.40X2-4.58X3.Conclusions There is significant difference for the serum NT-proBNP level among these groups.The clinical risks in severe asphyxia neonates are higher than others.The developing stage of the common diseases could be predicted by monitoring the prognostic equation of serum NT-proBNP level.
