摘　　要：目的研究外周血癌胚抗原(CEA)、生存素(Survivin)、基质金属蛋白酶-2(MMP-2)及金属蛋白酶抑制剂-2(TIMP-2)mRNA表达与乳腺癌的关系。方法采用逆转录实时定量聚合酶链反应(real-time PCR)检测54例乳腺癌患者和23名正常对照组外周血MMP-2 mRNA、TIMP-2 mRNA、CEAmRNA和Survivin mRNA的表达水平。以正常对照组结果为正常参考范围,分析乳腺癌患者外周血CEA mRNA和Survivin mRNA表达的阳性率。结果乳腺癌患者外周血可检测到不同水平的CEA mRNA、Survivin mRNA、MMP-2 mRNA和TIMP-2 mRNA的表达。Ⅰ/Ⅱ期及Ⅲ/Ⅳ期乳腺癌组外周血MMP-2 mRNA表达量高于正常对照组(P<0.01);TIMP-2/MMP-2比值则低于正常对照组(P<0.05、P<0.01);Ⅲ/Ⅳ期乳腺癌患者外周血CEAmRNA和Survivin mRNA表达量高于正常对照组(P<0.01)。Ⅲ/Ⅳ期乳腺癌患者外周血MMP-2 mRNA、CEA mRNA、Survivin mRNA表达量及TIMP-2/MMP-2比值与Ⅰ/Ⅱ期乳腺癌患者比较差异有统计学意义(P<0.01)。Ⅲ/Ⅳ期乳腺癌患者外周血CEAmRNA和Survivin mRNA表达的阳性率明显高于Ⅰ/Ⅱ期乳腺癌患者(P<0.01、P<0.05)。结论采用逆转录real-time PCR检测外周血CEA、Survivin、MMP-2 mRNA的表达以及TIMP-2/MMP-2比值可以成为一种无创性乳腺癌辅助诊断、分期、转移的方法。Objective To study the relationship of the mRNA expression of carcinoembryonic antigen（CEA）,Survivin,matrix metalloproteinase-2（MMP-2） and tissue inhibitor of metalloproteinase-2（TIMP-2） in peripheral blood with breast carcinoma.Methods The expressions levels of CEA mRNA,Survivin mRNA,MMP-2 mRNA and TIMP-2 mRNA in 54 patients with of breast carcinoma and 23 healthy controls were detected by reverse transcription real-time quantitive polymerase chain reaction（PCR）.The positive rates of the expressions of CEA mRNA and Survivin mRNA were analyzed,which referred to the results of controls as normal range.Results The different levels of the expression of CEA mRNA,Survivin mRNA,MMP-2 mRNA and TIMP-2 mRNA were detected in the peripheral blood of patients with breast carcinoma.The expressions of MMP-2 mRNA in breast carcinoma patients of stage Ⅰ/Ⅱand Ⅲ/Ⅳ were significantly higher than those in the control group（P0.01）.The ratio of TIMP-2/MMP-2 in breast carcinoma patients was significantly lower than that in the control group（P0.05,P0.01）.The expressions of CEA mRNA and Survivin mRNA in stage Ⅲ/Ⅳ patients were significantly higher than those in the control group（P0.01）.The difference of the expressions of MMP-2 mRNA,CEA mRNA,Survivin mRNA and the ratio of TIMP-2/MMP-2 between stage Ⅲ/Ⅳ patients and stageⅠ/Ⅱ patients had statistical significance（P0.01）.The positive rates of the expressions of CEA mRNA and Survivin mRNA in peripheral blood were significantly higher in stage Ⅲ/Ⅳ patients than in stage Ⅰ/Ⅱ patients（P0.01,P0.05）.Conclusions The reverse transcription real-time PCR for the mRNA expressions of CEA,Survivin and MMP-2 and the ratio of TIMP-2/MMP-2 in peripheral blood may be an assistant diagnosis method and can help for the diagnosis,staging and metastasis analysis of breast carcinoma in clinical practice.
