隐神经营养血管蒂逆行岛状皮瓣的临床应用

摘　　要：目的：报道隐神经营养血管蒂逆行岛状皮瓣临床应用的结果。方法：以隐神经相伴行的大隐静脉体表投影为轴线，根据受区所需要大小、部位及旋转点的位置设计送行岛状皮瓣。结果：临床应用9例，修复下肢远端创面，皮瓣8例成活，1例失败。皮瓣面积最大9cm×10cm。4例经随访1－2年，皮瓣感觉恢复正常．效果满意。结论：根据隐神经的血管构筑与皮肤止供密切相关的特点，设计隐神经营养血管蒂逆行岛状皮瓣安全可靠，操作简便，易于推广。具有临床应用价值。Objective: To report the reliable covenye of skin soft tissue defects in lower limb on distally based neurocutaneous island flaps of the saphenous nerve. Methods: Design of the reversed neorutaneous island flaps that cutaneous nerve as the anatomic landmark, based on the site of the flap rotation point and the size of soft tissue defects. ReSults:9 flaps were raised from the leg and all survived. Flap dimensions can be as large as 9cm ×8cm. The results were satisfactory, Four cases repair sensation after one to 2 years folow. ConcluSions: The vascularity of the saphenous nerve is closely connected with the vascularity of the skin.The flap whose blood supply by the vascular of the cutaneous nerve can be performed with a good results.
