摘 要:目的 评价无器质性心脏病者QT离散度(QTD)与室性心律失常的关系。方法 于DCG检查前进行常规12 导ECG检查,测量QT间期,计算QTD,QTcd(校正的QTD),根据24 小时ECG检出的室性心律失常情况,分为无室早组,室早< 100 次,室早≥100 次组,成对室早组及无成对室早组,比较各组病人之QTD,QTcD。结果 无室早组,室早< 100次,室早≥100次组,成对室早组及无成对室早组之QTD分别为28.9±11.1m s,29.5±12.6m s,27.8±11.8m s,29.1±9.3m s,28.9±10.9m s.QTcD分别为35.3±13.2m s,35.8±14.0m s,36.2±13.7m s,36.5±12.9m s,35.7±11.4m s,各组病人之QTD,QTcD比较均无显著差异,P> 0.05。结论 无器质性心脏病者QTD,QTcD与室性早搏的多少及有无成对室早无关。Objective To evaluate the relationship of between QT dispersion(QTD) and ventricular arrhythmias.Methods 100 consective cases of patients suspected of arrhythmias without organic heart diseases were analyzed.12 lead electrocardiogram(ECG) was recorded before 24 hour dynamic electrocardiogram(DCG) recording.QTD and QTcD were defined as maximal QT -minimal QT and maximal QTc -minimal QTc respectively.Patients were divided into 5 groups based on the presence or absence of ventricular arrhythmias,group A,no ventricular premature contraction(PVC),group B,PVCs less than 100 beats per 24 hours,group C,PVCs at least 100 beats,group D,with couple PVCs,group E,with PVCs but without couplets.Results QTD was 28.9±11.1ms,29.5±12.6ms,27.8± 11.8ms,29.1±9.3ms,28.9±10.9ms in group A,group B,group C,group D and group E respectively,QTcD was 35.3±13.2ms,35.8±14.0ms,36.2±13.7ms,36.5±12.9ms,35.7±11.4ms in each group gespectively.No significant differences of QTD and QTcD were found between any two groups( P > 0.05).Conclusions There was no relationship between QTD(and QTcD) and ventricular arrhythmias in patients without organic heart diseases.
